(We ask that you pick your pet up between 12 pm and 2 pm, so that we may feed, exercise, and bathe your pet.)
Picking up your pet after 2:00 pm will result in an additional day’s charge for boarding.

Cost is determined by breed, size and coat length.
For your pet’s health and safety, all pets must be up to date on their vaccinations. Proof of vaccinations must be provided at check-in. If you are unable to provide this proof, we will vaccinate your pet(s) at your expense in accordance with our vaccination policy. Dogs must be up to date on Rabies, Distemper (DHP), Parvo, Canine Influenza and Bordetella vaccines, and free of intestinal parasites. Cats must be up to date on Rabies and Feline Distemper and Upper Respiratory vaccines and free of intestinal parasites. Pets showing visible fleas and ticks will receive a flea and tick treatment at the owner’s expense.
One of the advantages of boarding with us is that veterinary attention is readily available should the need arise. If your pet becomes ill, we will call the emergency number(s) listed above regarding symptoms, treatment options, and estimates of additional costs. If no one can be reached, please indicate your wishes below should your pet(s) require treatment to relieve immediate discomfort or resolve a serious medical condition.

●I understand that the latest pick up time for boarding is 5:30pm Monday, Wednesday, Thursday, and Friday. The latest pick up time is 6:30pm on Tuesday. And the latest pick up time is 11:30am on Saturday.

All animals are boarded separately unless expressly requested by the owner. Switzerland Animal Hospital cannot be held responsible for any injury or loss to your pet(s), or for medical expenses that arise due to fighting among pets being boarded in the same kennel.

In the unlikely event of a natural disaster such as a hurricane or act of God, Switzerland Animal Hospital, cannot make any express guarantees nor will be held responsible for any injury or loss to your pet(s) while boarding at Switzerland Animal Hospital. We will provide and exercise great care while your pet is at our facility but cannot guarantee your pet’s safety during any event out of our control.
I HAVE READ AND UNDERSTAND THIS AGREEMENT. I fully intend to pick up my pet(s) on the above specified date and time. If circumstances change, I will notify Switzerland Animal Hospital.
