Berdex –Collection Documentation Requirements

Collection documents to be sent via courier to:

Documents Requirements:

1)  Three (3) originals and three (3) copies signed Commercial Invoice showing Packer’s name and address and FDA Registration Number

2)  One (1) original and three (3) copies of Certificate of Origin

3)  One (1) original and three (3) copies of Packing List

4)  Beneficiary’s Certificate confirming that one set of non-negotiable documents was sent by courier to Berdex Seafood, 111 Pine Street, Suite 1615, San Francisco, CA 94111, USA, Tel: 415-391-6700 Attn.: Gregory Thenn, Traffic Manager

5)  Quality inspection certificate issued and signed by Berdex purchasing agent, stating that cargo has been inspected and was found to be of good merchantable quality with weights, counts, packaging- and product specifications as per contract

6)  One (1) original and three (3) copies of US Department of State shrimp Exporter’s/Importer’s Declaration (Form DS-2031)

7)  Beneficiary’s certificate stating that in the event of an FDA rejection seller agrees to reimburse buyer 100% of Invoice Value including all related importation costs and return freight charges via telegraphic transfer upon presentation of documentation covering said rejection.

8)  Beneficiary’s certificate showing all lot numbers, lot quantities and production date of each lot stating that:

-The Beneficiary’s plant (s) is in compliance with the USFDA’s HACCP regulation 21 CFR part 123 and that product exported are processed in compliance with the good manufacturing practices and standard sanitation operating procedures.

The following steps have been taken for the lots exported:

A-  Hazard Analysis has been conducted by the HACCP team at the Beneficiary’s plant (s)

B-  HACCP Plan with critical control points has been developed and monitoring and record keeping at these critical control points occurred during production of all product packed and produced in the Beneficiary’s plant (s)

C-  Beneficiary’s plant has produced a written sanitation standard operating procedure (SSOP), sanitation monitoring and record keeping, consistent with the USFDA HACCP regulation occurred during processing.

9)  Pre shipment survey certificate issued by SGS, Lloyds, MCI or Indra Surindo or their agent(s) showing:

-  Container and seal numbers

-  Quantity, pack and description of goods including lot numbers and quantity in each lot

-  External and internal condition of container (digital pictures preferably)

-  Internal temperatures of container prior to, during and at completion of loading the container

10) One (1) Facsimile copy or photocopy of relevant Berdex Purchase Order signed by Applicant and Beneficiary.

11) Beneficiary’s Certificate stating that the detected Chloramphenicol level is less than 0.3 PPB

12) Continuing guarantee signed by Beneficiary stating: This is to verify that (name of Beneficiary) in the production of raw frozen seafood is operating under HACCP and sanitary programs that are in accordance with the USFDA’s Seafood HACCP regulation 21 CFR 123.

13) Original Health Certificate of Fish and Fishery Product issued by the Country of Origin, Ministry of Marine Affairs and Fisheries.

14) Full set originals and three (3) non-negotiable copies clean on board Ocean Bills of Lading consigned to Wells Fargo/HSBC Trade Bank marked freight and destination charges prepaid, Notify Party must be shown as:

-For deliveries to US East Coast: JC Murray, 200 Route 22 East, Hillside, NJ 07205, Tel: 973-282-2262, Attn.: Mike Murano

Prepaid freight charges must be shown on the Bill of Lading.

Other Terms and Conditions

1)  Berdex Purchase Order number (for example: P-46520) must be shown on all shipping documents.

2)  Container number must be shown on all documents except copy of Purchase Order and DS-2031.

3)  All documents to be issued in English

4)  Sample of “Tiger Bay” bags in compliance with www.tigerbay.net latest designs accessible with username Berdex and password Sansome01 with correct UPC codes to be sent with non negotiable documents to Berdex by courier.

For shipments from Indonesia, Traceability Documents evidencing that products are not re-exported Chinese shrimp, must be supplied showing the following:

1)  Incoming date of raw material and description of raw material

2)  Raw material Supplier’s name

3)  Raw material supplier’s Lot number

4)  Quantity of raw material delivered (arranged by lot number)

5)  Packer’s lot number

6)  Packer’s Production date

7)  Packer’s Product description

8)  Packer’s Product Size/lb
