INVITATION TO PRE-QUALIFY:

Kearns Improvement District is seeking contractors that would like to pre-qualify to submit bids for the construction of the 5 MG Culinary Water Tank and 30-inch Diameter Culinary Waterline Project.

The scope of work is anticipated to include the following work items:

The construction of a water-tight reinforced concrete structure with a 5 million gallon capacity; valve vault with piping, valves, etc.; 30-Inch diameter inlet/outlet piping; site work; grading, and emergency overflow piping and structures. The project also includes electrical connections to and electrical work associated with the valve vault at the tank site.

No pre-submittal information meeting will be held.

The qualifying contractors must:

1)Show a minimum of 5-years of experience installing reinforced concrete culinary water tanks.

2)Demonstrate construction of at least three (3) reinforced concrete culinary water tanks with a capacity of 3 Million Gallons or largerand construction of at least three (3) culinary waterlines 18 inches in diameter or larger within City Streets in the last 10 years.

3)Document completion of 3 separate projects with a minimum cost of $1,000,000 each. The total value of change orders for each of these projects must equal less than 10% of the total project cost.

4)Demonstrate through references satisfactory performance on similar projects.

5)Have a local office in the State of Utah.

6)Provide a minimum of 5 references.

7)Show capability to meet the OWNER’S insurance and bonding requirements.

Only Contractors who are pre-qualified by Kearns Improvement District will be allowed to submit bids for the project.

Information provided by contractors will be sent to Kearns Improvement District appointed Selection Committee for evaluation. Only those contractors that are selected by the Selection Committee will be sent the Request for Proposal for the Construction of the Kearns Improvement District5 MG Culinary Water Tank and 30-Iinch Diameter Culinary Waterline.

SUPPLEMENTARY CONDITIONS

SC-5.04CONTRACTOR'S LIABILITY INSURANCE

The requirements of General Conditions paragraph 5.04 for insurance to be purchased and maintained by the CONTRACTOR, and any renewals thereof, are modified and supplemented as follows:

C.A copy of the acceptable Certificates of Insurance filed with the OWNER shall be delivered to the ENGINEER for review before any work at the site is started.

D.All liability insurance policies for the project shall include an endorsement naming the OWNER and ENGINEER as additional insured.

E.The CONTRACTOR General Liability Insurance shall be comprehensive form and shall include the following coverages:

1.Premises/Operations, collapse hazard, underground hazard, products/completed operations hazard, contractual insurance applicable to CONTRACTOR’s obligations under SC 6.03 Indemnification, broad form property damage, independent contractors, and personal injury.

2.Auto Liability Insurance shall be comprehensive form and shall cover owned, hired and non-owned vehicles.

3.The limits of liability for the insurance required by paragraph 5.04 of the General Conditions shall provide the following coverages for not less than the following amounts or greater where required by Laws and Regulations:

a.Workers' Compensation, etc. under paragraphs 5.04.A.1 and A.2. of the General Conditions:
