PARENTAL AGREEMENT

1. All children must have physical exams and be current with all necessary immunizations. This is State mandated and due the first week of school. No child will be permitted to begin school without this completed form.

2. Please inform the school IMMEDIATELY if your child or a member of your family has a contagious disease, including lice.

3. Please do not send your child to school with a runny nose cough or cold.

4. If there is a major change in your child’s routine, please keep the teacher informed.

5. Our school year and calendar is in accordance with the Board of Jewish Education, SAJES, HuntingtonSchool District #3. Snow closings also follow the HuntingtonSchool District #3. We reserve the right to close school at the Temple’s discretion even if HuntingtonSchool District #3 is open to maintain the safety of our students and our staff.

6. We are State mandated to report any suspicion of child abuse or neglect.

7. In order to keep children safe, we feel that biting must be treated with a strong message to the young child who bit someone else. If a child bites more than 2 times, that child will be sent home with the possibility of being asked to leave the school.

8. Please be prompt for pick-up and drop-off. It is upsetting to your child to have to wait with worry as to your whereabouts.

9. Please give the teacher a carpool list and name or picture of pick up person if other than a class parent.

10. We recommend clothing that is for “work & play”. Sneakers are preferred over dress shoes.

11. Each child should have a complete change of clothes that are labeled in his/her backpack. Please provide a container of anti-bacterial soap,2 boxes of tissues, and a box of wipes. If your child is not toilet trained, please include diapers and their own wipes readily available in a plastic bag in their backpacks.

12. A sign up sheet will be posted at the classroom door for weekly Shabbat celebrations. You will be notified in advance of school parties and special programs.

13. We will not be responsible for handing out birthday invitations. It is recommended that parents should mail invitations from home. If your choose to hand them out during school, there must be an invitation for every child in the class, and because we are a Conservative Temple, then the party should not take place during Shabbat.

14. Tuition payments are payable monthly and are due by the 10th of the month. Special financial arrangements must me made with the bookkeeper in advance. All tuition payments must be current in order to maintain your child’s attendance in our program.

15. In order to comply with the dietary standards of our Synagogue, all lunches must be DAIRY.

16. I give permission for my child in the 3 and 4 year old classes to take short supervised walks in the school area.

THIS AGREEMENT HAD BEEN INSTITUTED TO ASSURE THAT YOUR CHILD HAS A SAFE AND ENRICHING EXPERIENCE AT HUNTINGTON JEWISH CENTERNURSERY SCHOOL.
