TO: All Prospective Proposers

SUBJECT: Request for Proposal # 111016

Hazardous Waste Collection, Transport & Disposal from Dane County Clean Sweep Facility at 2302 Fish Hatchery Road, Madison, WI 53713

The following addendum becomes a part of the above referenced RFP. All other terms and conditions remain in effect, unchanged.

The following is a listing of clarification answers to questions received up until February 14, 2011

1. Under the technical requirements section on page 15 item 5.1A 1 there is a statement “Dane County waste shall not be mixed with waste from any other facility”, is this requirement intended to be until receipt and acceptance at the processing/TSDF facility?

Yes, the Dane County waste must be kept separate from other waste until receipt & acceptance at the processing/TSDF facility.

2. What is the collection procedure for the controlled pharmaceuticals?

Local law enforcement oversees the collection of controlled substances at collection events and at collection boxes located on their properties. Controlled substances are removed from packaging (to the extent possible) and placed into 5 gallon plastic drums with screw-type lids. Contractor and its employees are not expected to directly participate in the actual collection of controlled substances. The Contractor’s role will be to; 1) provide the vehicle and personnel necessary to facilitate the transportation of materials from Dane County/local law enforcement secure storage facilities to the disposal facility under local law enforcement escort, and 2) to facilitate a witness burn of such material.

3. Do you have a DEA registration number that can be used for shipment of the controlled substances?

To the best of our knowledge, since the materials are collected from households and under the control of local law enforcement, a DEA registration number is not required.

4. Where and for how long are the controlled pharmaceutics stored, and are they stored at multiple locations or a single location?

The duration of storage and storage locations for controlled pharmaceuticals is uncertain at this time. At this time there are only two drop-box locations in Dane County and one community collection event planned for 2011. The number of drop box locations is expected to increase as the program develops. At some point, community collection events may be phased out of the program. For the purposes of this RFP, assume that pharmaceuticals will be consolidated at a single local law enforcement facility in Dane County prior to transportation, and shipment to the disposal facility will occur on a frequency of once or twice per year. Shipping/transportation pricing should be on a cost per event basis, and these unit costs will be used if additional shipments to the disposal facility are necessary.

5. At what frequency are the controlled pharmaceuticals shipped?

See Answer to Question No. 4, above.

6. Since the shipping of the controlled pharmaceuticals is a significant cost, could you add a separate line covering the shipping per load and another for the witness burn?

Yes, REVISED Schedule A (REVISED Attachment F) is attached and reflects the following cost items:

·  Shipment/transportation cost per event

·  Disposal cost per pound

·  Witness burn cost per event

7. Are the controlled pharmaceuticals currently handled under another contract such as the State of Wisconsin Contract; and if yes, should they be excluded from the cost evaluation if they will be handled through another contracting vehicle?

You are not required to provide cost data for controlled pharmaceuticals. These costs will not be factored into the evaluation of costs for award of the contract. However, bidders are strongly encouraged to provide cost data for pharmaceutical disposal.

8. Mail back programs are available from Class II through V controlled pharmaceuticals; would this method be acceptable to you?

Currently, mail-back programs are not acceptable. However, they might be at some point in the future. You may include costs for mail-back programs as an option, but any controlled substance costs will not be factored into the evaluation of costs for award of the contract.

9. Is waste to energy an acceptable handling method for the latex paint?

Currently, waste-to-energy is not an acceptable method for handling latex paint. However, it might be at some point in the future. You may include costs for waste-to-energy as an option, but this and other alternatives will not be considered in the award of the contract.

10. Are cylinders covered under this contract, and how should we provide pricing for them if they are?

Yes, REVISED Schedule A (REVISED Attachment F) is attached and reflects the cost of cylinder disposal.

11. When and under what circumstances do you order the analytical services?

Typical circumstances where analytical services may be ordered include events where unknown drums or containers are discovered in public or private locations in Dane County and additional information is needed before the container can be safely moved. Incidents of this nature are a relatively uncommon occurrence in Dane County.

12. How often and in what situations are offsite pickups required?

Rarely. Under normal circumstance, if owner or responsible party is identified, they will be required to either transport to the HHW facility or instructed to contract directly with a hazardous waste contractor for disposal. For the purposes of this RFP, this type of off-site pickup is considered an unanticipated, rare and unique event. The only “anticipated” off-site pickups are those associated with the conductance of satellite HHW collection events.

13. What materials would the contract be required to package?

At satellite events, the Contractor is required to package all materials prior to shipment. This contract requires the Contractor to package all materials received at the HHW site. However, under normal circumstances, County personnel will package most of the waste materials received at the HHW facility. For the purposes of the RFP, contractor should assume that personnel will have to spend an average of 1.25 hours packaging unknown materials and miscellaneous labpacks 8 times per year during pickups from the HHW facility, for a total of 10 hours per year.

We (I) propose to pack (as required) and transport from the Dane County Clean Sweep Facility at 2302 Fish Hatchery Road, in Madison, Wisconsin, or satellite collection sites, containers of hazardous wastes listed in this RFP proposal; to see that such materials are used, recycled, or disposed of in accordance with WDNR and EPA regulations; and to furnish the labor, equipment, tools, expertise, insurance, and permits required to complete the work.

REVISED Schedule A shall apply where the entire scope of work is awarded in a single contract and shall also apply where the scope of work is split into multiple contracts. All primary disposal facilities shall be stated in Schedule B of this proposal.

Quantities listed on this form are estimates based on a normal 6 month program season. These quantities are estimates for planning purposes only and are not guaranteed.

·  ** Absorbents from waste oil clean-ups must be recycled in accordance with Wisconsin Act 86, which prohibits disposal of used oil filters and oil absorbent materials in a solid waste disposal facility, amends Chapter 287 and takes effect on January 1, 2011. The amendment adds used oil filters and oil absorbent materials to the list of items banned from land disposal (ch.287.07(4m)).
