This checklist gives the interviewer a list of required actions for an AFROTC High School Scholarship Program (HSSP) interview.

Ensure the applicant is briefed on the following items and that the
appropriate listed action was accomplished after the interview:

1.  The HSSP interview was accomplished with applicant alone (without parents, legal guardians, mentors or friends.)
______
2.  The applicant was briefed on USAF policies regarding civil involvement, drug and alcohol abuse.
______
3.  Applicant was advised that any future use or experimentation of drugs, alcohol abuse and any additional civil involvements could make them ineligible to activate or retain a scholarship or obtain a commission.

4.  The AFROTC IMT 35 was submitted to HQ AFROTC/RRUC. If corroboration is required, AFROTC IMT 4 must be submitted with the IMT 35, and the AFROTC IMT 14 must also be completed and returned to AFROTC/RRUC with the results of the inquiry corroboration. The AF IMT 2030 was submitted to HQ AFROTC/RRUC. Ensure applicant initials appropriate blocks and if any question pertaining to drugs usage was answered “yes” have the applicant complete the remarks by answering: the following: dates and circumstances on each occurrence to include effects resulted, specify drugs used, method used, residual effects, current feelings towards drugs use, have the applicant signs and date. Ensure the witness block is signed. All forms may be faxed or scanned and emailed to AFROTC/RRUC.

5.  The applicant was advised of DOD policy concerning homosexual conduct.
______
6.  Applicant was briefed that enrollment in the specific academic major in which the scholarship is offered is required, and that scholarships are offered based on the needs of the Air Force. Changes can be submitted prior to the board via email to the appropriate HQ/RRUC technician.

7.  Review Applicant’s major and school choices to ensure they are still current. If they want to change majors or school, have them complete a “Major and/or School Choice Change” request found in this book and fax to HQ AFROTC/RRUC. THIS IS A MANDATORY ITEM!

8.  Applicant was informed of weight/body fat and physical fitness standards required for activation.

9.  Applicant was briefed on the types and lengths of scholarships available.

10.  Applicant was briefed on marriage and dependency limitations.

11.  Applicant was advised he/she must be enrolled in AFROTC classes and Leadership Laboratory beginning with the first term of their freshman year regardless of the length of the offer.

12.  Applicant’s height, weight, and maximum allowable weight were recorded on the AF Form 4060.

13.  The military service of a parent or guardian, and the evaluation data on the AF Form 4060 is complete.

14.  The rater’s interview history was annotated on the AF Form 4060

15.  The AF Form 4060 was completed and forwarded to HQ AFROTC/RRUC via ALOWeb. NOTE: The 4060 is required even if the commander offers a CLS.

16.  Originals of the AF Form 4060, AFROTC IMT 35, and AF IMT 2030 must be retained at the detachment for two (2) years.
