PERFORMANCEAUDIT IN DIAGNOSTIC COLONOSCOPY –YEAR 2016 IN REVIEW IN A UNIVERSITY ENDOSCOPIC CENTER

Background and Aim: Colonoscopy, the gold standard evaluation method for the colon should be delivered by endoscopists performing high quality procedures.By some authors, a good quality indicator of the performance at colonoscopy in an advanced endoscopy center is achieving a cecal intubation rate of over 90%.Theaim of this study was to assess the performance at colonoscopy in the Endoscopy Unit from the Department of Gastroenterology Timişoara in the year 2016, a university center that also trains fellows in gastroenterology.
Material and methods: we studied all the colonoscopies performed in our departmentin the year 2016, analyzing the percentage of total colonoscopies.We have analyzed only the colonoscopieswith the intent of total colonoscopy,excluding: endoscopic reevaluations,impossible to surpass stenosis.The colonoscopies were performed either by senior endoscopists or by final year fellows in training in gastroenterology.
Results: the number of colonoscopies (with the intention of total colonoscopy) performed in 2016 was 740.In 2016 out of 740 colonoscopies, 718 were total (97.02%).In this group, total colonoscopy could not be performed in 22 patients because of inadequate bowel cleansing, tortuosity or for unknown anatomical reasons.
Conclusion:In a Clinical Department of Endoscopy, continuous audit of colonoscopy performancemaintained the cecal intubation rate 90%, even if colonoscopy was performed by several endoscopists (including fellows in training).
AUDITAREA PERFORMANŢELOR LA COLONOSCOPIA DIAGNOSTICĂ – UN REVIEW AL ANULUI 2016 ÎNTR-UN SERVICIU UNIVERSITAR DE ENDOSCOPIE

Introducere și scop: Colonoscopia, metoda gold standard de evaluare a colonului ar trebui efectuată de către endoscopiști cu indice de performanță ridicat.După unii autori, un bun indicator calitativ al performanței endoscopice într-un centru de endoscopie avansat este reprezentat de atingerea unei rate de intubație cecală de peste 90%.Scopul acestui studiu a fost evaluarea performanţelor la colonoscopie la nivelulServiciului de Endoscopie al Clinicii de Gastroenterologie Timişoara în anul 2016, serviciu universitar care pregăteşte şi rezidenţi în gastroenterologie.
Material și Metodă: am studiat toate colonoscopiile efectuate în departamentul nostru pe anul 2016, analizând procentul de colonoscopii totale.Am analizat numai colonoscopiile cu intenție de colonoscopie totală, excluzând: reevaluarile endoscopice, stenozele nepasabile.Colonoscopiile au fost efectuate atât de endoscopiști experimentați, cât și de rezidenți aflați în ultimul an de pregătire.
Rezultate: numărul de colonoscopii (cu intenție de colonoscopie totală) efectuate în 2016 a fost de 740.În 2016 dintre cele 740 de colonoscopii, 718 au fost totale (97.02%) .La acest grup colonoscopia totală nu a putut fi realizată la 22 de pacienți datorită pregătirii colonice inadecvate, sinuozității sau din considerente anatomice necunoscute.
Concluzii: într-un Department Clinic de Endoscopie, auditarea permanentă aperformanţelor la colonoscopie a menţinut o rată de intubaţie cecală de peste 90%, chiardacă colonoscopia a fost realizată de mai mulţi endoscopişti (inclusiv rezidenţi degastroenterologie).
