ACKNOWLEDGMENT AND ATTESTATION FORM

The School District of Philadelphia (SDP)willserve as the administrative sponsor for several applicants to the PHLpreK Continuation Application process. As a sponsor, SDP takes full responsibility for ensuring the applicants meet the necessary eligibility requirements and understand fully the responsibilities and services associated with the Continuation Application process and PHLpreK.

The School District of Philadelphia does hereby affirm and expressly declare confirmation, certification, and assurance of the followingon behalf of all preK sites for which it applies:

1)The Applicant site has read the entire Continuation Application packet and agrees to abide by all requirements stated therein.

2)The Applicant site has read and intends to comply with the City of Philadelphia General Provisions.

3)The information detailed in the application submitted in response to the Continuation Application is accurate.

4)SDP and the Applicantsite acknowledgesthat this Continuation Application is strictly for the purpose of gaining knowledge of services and expertise available and is not a commitment or promise of a service award from PHMC.

5)SDP acknowledges that a response to this Continuation Application must be submitted in alignment with the timeline noted in order to be eligible for consideration.

6)The Applicant site attests that they are in good standing with their local government (City of Philadelphia), the Commonwealth of Pennsylvania and the federal government and are compliant with all applicable tax obligations at the time of application.

7)Applicant sites, including providers and subcontractors working with providers who are funded through the City, are expected to follow the City of Philadelphia Twenty-First Century Living Wage and Benefits Ordinance.

8)SDP and the Applicant site authorizes the City, PHMC and designated parties to review agency fiscal and program data to determine impacts of the program and inform research and understanding. Provider information will not be made public but aggregated, de-identified data will be shared publicly.
