House of New Hope will provide Mental Health Assessment Services, non-physician services to children and adolescents, birth through age eighteen (18), receiving who meet criteria for mental health services and adults. The Mental Health Assessment service is provided on a face-to-face basis by qualified providers according to Chapter 5122-29-04 of the Ohio Administrative Code.
Mental Health Assessment is a clinical evaluation provided by an eligible person prior to the initiation of any mental health services except in the condition of the need to deliver a crisis or emergency intervention, in response to treatment, or when significant changes occur. It is a process of gathering information to assess client needs and functioning in order to determine appropriate service/intervention based on identification of the presenting problem, evaluation of mental status, and formulation of a diagnostic impression.
The outcome of mental health assessment is to determine the need for care and recommend appropriate services/treatment and/or the need for further assessment. Results of the mental health assessment shall be shared with the client/family (as is age appropriate).
Mental Health Assessment service involves appropriate and timely collateral contacts with family, parent or guardian and/or other providers or agencies servicing that youth or family. Qualified providers are sensitive and responsive to issues related to cultural diversity and socio-economic factors.
1.The following Mental Health Assessment activities will be provided by qualified personnel according to Chapter 5122:29-04 of the Ohio Administrative Code. Youth will be assessed by a Licensed Social Worker and/or Licensed Professional Counselor, with clinical sign-off by an independently licensed professional.
2.Components of the Mental Health Assessment shall include at a minimum:
a.An age appropriate psychosocial history and assessment, to include consideration of multicultural/ethnic influences;
b.The presenting problem;
c.A diagnostic impression/interpretive summary and treatment recommendations;
d.As determined by the provider, any other clinically indicated areas.
e.Clinically related areas of assessment shall include components as defined by CARF standards (see HONH’s Consumer Assessment Procedure for a listing of specific components.)
