Nominations for all award categories may be individual submission
or by nomination. Each award is based on calendar year.

A USPTA professional-level member residing in the Midwest Division since June 2013 may apply for one or more of the awards. Nominee must be a current certified professional member of the USPTA Midwest Division in good standing. Recreational Coaches may apply with Professional-level members for the Lesson for Life award for either under or over $10,000. Facility Manager does not have to be USPTA member.

-Presented to the Midwest member contributing the most to the USPTA in their first year of active service / Fay Tooley Memorial Award
-Presented to the Midwest member who has done the most to promote the USPTA and tennis but has not received notoriety.

/ The award will be divided into two subcategories to honor the various ways USPTA professionals contribute to charities in their area, events raising less than $10,000 and events raising more than $10,000. An event may win the award only once in a particular category.
Recreational Coach may apply for each Charity Event of the Year award

  4. Individuals may submit scrapbooks that include this application.

We encourage applicants to be prepared to attend the
USPTA Midwest Division Conference if they receive the award.

Nancy Mickler Award

-Presented to the Midwest woman member who has done the most to promote
tennis within the Midwest.

-Should an individual move to another state within the Midwest Division, the state where he/she has resided longest is considered for the award selection.

-Applicants must score at least 12 points to receive this award. If no one in a particular state has scored the 12 points then the award will not be given that year.

-An individual may receive this award more than once.

-This award need not be given every year.

  1. Contributions to the USPTA (0-5 points): List anything to do with local, Midwest, or National USPTA. Could include Lessons for Life clinics, Tennis Across America, Little Tennis programs, USPTA Leagues, USPTA Champion of Champions event, USPTA coaches clinics, USPTA tournament, USPTA Junior Circuit and use of CourtsideUSA, plus other USPTA endorsees. Also, USPTA committees or board that you serve or other USPTA events found on the website.
  2. Contributions to local tennis organizations (0-3 points): List anything that has to do with local USTA, Community Tennis Associations, parks and recreation, schools, etc.
  3. Club Achievements (0-3 points): List successful programs at your clubs, number of players in club programs, tournaments you host.
    Note: Any USPTA programs that you run should be listed in area 1 – Contributions to USPTA. Fundraisers of any kind, count as a Lessons for Life program and should also be listed in area 1 – Contributions to USPTA.
  4. Playing Record, minimum of 2 events (0-1 point). These may include Pro-Ams, tournaments, USTA or USPTA leagues, exhibitions, or any other playing event.
  5. Career Development (0-3 points): List any conferences, workshops, Specialty Courses, seminars, Specialists Degrees earned, or any other education events. Include non-USPTA courses, too.
  6. Volunteer work (0-3 points): List all activities you are involved in to promote tennis for which you are not paid.
  7. Professional Rating (1-4 points): A point value will be given based on professional rating: Master Pro = 4 pts., P1 = 3 pts., P2 = 2 pts., P3 = 1pt. Please list your current level of certification.
  8. List any additional information pertaining to the Nancy Mickler Award which seeks women have done the most to promote tennis within the Midwest. This information will be used as the tiebreak.
