REGISTRATION FOR FELLOWSHIP LUNCH

IN PLACE OF OUR REGULAR LUNCHEON: WEDNESDAY, JULY 6, 2016

In place of our regular luncheon on Wednesday, July 6, we have volunteers who have agreed to hold a breakout luncheon in their offices or conference room. The listing of locations for these breakout luncheons is listed on the next page. If you would like to attend one of these luncheons, youmust signup in advance. Registration is required so we know how many to plan for food and space at each of the locations. Attending one of the luncheons will credit your weekly attendance, and there will be an attendance sign-in sheet at each site. Lunch will be provided at no cost to pre-paid members as this is part of the regular meal plan payment. Note: If you plan to bring a guest, please also add that individual’s name to your registration form. Any guests must be registered in advance of the luncheon.

Please complete and PRINT YOUR NAME on the top of the registration form on the next page and send to:
