THIS IS A REQUEST TO BRING A GUEST TO A DANCE THAT IS NOT CURRENTLY ATTENDING AYALA HIGH SCHOOL. This request must be filled out completely, signed by an administration from the guest’s high school, photocopy of ID attached and delivered/faxed to Ayala High School Administration.

Ayala High School Administration reserves the right to refuse admittance to any dance guest who does not currently attend AyalaHigh School.
If High School age, the guest must be enrolled in school and the guest’s school administration must sign the guest pass.
Age reguirements: Guests must be no younger than 9th grade and not older than 21 years of age.
All guests must present a current valid picture ID. If high school student, it must be their student ID. Non high school students may use a driver’s license.
Ayala students are limited to one guest per dance.
The Bulldog Dress Code is in effect including the following items for Homecoming:
Homecomingis a semi-formal dance. No revealing clothing may be worn including clothing where excessive cleavage is displayed. Backless is okay as long as no posterior is exposed. No clothing cut down to the navel or below the top of the breast line. Dresses are not to be shorter than mid-thigh. Gentlemen are not allowed to go shirtless.
The following behavior guidelines are in effect for Homecoming:
Behavior on and off the dance floor must be socially acceptable and adhere to the Behavior Guidelines of Ayala High School and the ChinoValleyUnifiedSchool District. Dance movements and mannerisms must reflect the standards of modesty and “good taste.” Lewd and/or lascivious dancing known as “freaking” will not be tolerated. Front to back dancing
Dirty Dancing, Grinding or “Freak” dancing
Straddling legs
Bending over
Touching of breasts, buttocks or genitals
Making out (no overt and/or prolonged public displays of affection)
Individuals, couples, or groups engaging in such acts will be stopped and warned on the first occasion and have their wristband marked and sit out in a designated area. On the second occasion, parents will be contacted and students may be asked to leave the dance, without refund, and properly disciplined by school standards.
The following items will not be allowed and will be taken away and not returned.

Every student attending the dance will be searched upon arrival. Breathalyzers will be on site.
All students must arrive by 9:00pm. No students will be admitted after that time.
Violations of the behavior or dress code guidelines will result in removal from the activity. No refund will be given. Parents will be notified and expected to pick up their student.
