What is senior ulti-care?

The Senior Ulti-Care group was created to give seniors access to compassionate, vetted and trusted experts in the field of senior care. Whenever you work with a client we owe it to our clients to think beyond the services we provide but to think how we can further the quality of life of our clients through services our partners offer.

The group will consist of consummate professionals in non-competitive fields who will learn about each other businesses as to better be able to recognize when a senior could benefit from the services that the group provides with the commitment to introduce the senior to the provider within in group.

What services do you offer?

We offer highly detailed personalized concierge care management services.

Describe what is NOT a great client/referral for you?

An older adult or family without the ability to pay for private services.

What geographical area do you serve?

North County primarily, but we will go to Rancho Bernardo, Del Mar, La Jolla, and Solana Beach.

What are great questions to ask our clients to find out whether they would be a great client for you?

Most often the people you will encounter that may be needing our service but not realize it are the adult children of older adults. These questions pertain to them.

How are the challenges of caring for an older adult parent impacting your life? Could you benefit from some help?

Do you worry about the quality of care your parent is receiving in their own home, assisted living, or nursing home?

How would you like to learn about how the quality of life of your parent can improve?

Do you want mom or dad to stay home but don’t know what they need to do so and or how to make sure they are continually getting what they need as their level of care requirements change?

Are you feeling overwhelmed by the care of your parent and uncertain about the choices you are making on their behalf?

Kindred Hospice

What services do you offer?

Kindred Hospice offers medical, emotional, and spiritual support for patients in their home, skilled nursing facility, assisted living or independent facility who have a terminal illness...and the physician has deemed them having a 6 months or less prognosis should the disease run its normal course.

This doesn't mean they have to pass in those 6 months but that's the starting point. As long as they qualify when they are due for their recertification they can remain on hospice. If they get better, they graduate and come off hospice. They can come back on when they decline again.

The more common diagnosis are:

Alzheimer's/dementia

End stage stroke/cardiac/pulmonary/liver/ renal disease

Cancer

A decline in ADL and function add to the qualification for hospice as well as recurrent infections, falls, weight loss and more...

Hospice Medical Director, although their PCP can follow as the attending physician

Medication having to do with the hospice diagnosis are covered, as long as they aren't aggressive treatment and are life sustaining

Medical equipment provided, same day delivery

Admission can happen in as little as 3 hrs

We can admit and are available 24/7

We take care of the caregiver as well as the patient

Hospice is used to maintain an optimal qualify of life, pain and symptom free

***I tend to say it nicely but if someone is terminal, they are going to pass whether or not they are on hospice. We do not aid in dying and people have full control over their medication usage.

What makes your business different from your competitors?

We take any and all insurances. We consider non-funded on a case by case and have responses for them fairly quickly.

We are nationwide and can facilitate transfers across the state or country as well.

If the patient or loved one calls in the middle of the night, weekends or holidays as well, a nurse will answer, not an answering service.

I like to follow my referrals from the hospice presentation to the patient and family, to obtaining the order and medical records, yo the admission.

Our company has excellent communication and welcome any feedback that allows us to address any concerns, so we may rectify them and meet your expectations.

Describe the profile of your clients

You don't have to qualify a patient. If you know they have had a significant decline in function (previously used a cane and now are in a wheelchair; now they are incontinent; depend on assistance with a few or most ADLs), have exhausted any further aggressive treatment; opted not to continue treatment or the physician has declined further treatment of its more harmful than good to continue with it.

Describe what a good client/referral for you looks like

A good client/referral is one who has been prepared for the hospice discussion. If they aren't aware of the purpose of my visit, it will make them upset and feel ambushed. If you have difficulties speaking about death and dying, it'd be difficult to speak about hospice. That is my specialty and I can have that conversation with or without you present, whichever makes you most comfortable.

You don't have to qualify patients, it helps if you recognize the change in ADL performance, the caregiver is needing assistance or needs a break, and don't be afraid of bringing up the subject.

As long as they meet criteria, they can remain on hospice. Pulmonary and cardiac patients can last a long time and receive hospice support. If they are afraid of the word "hospice" we can call it palliative care or Kindred Healthcare.

It's been documented that patient on hospice tend to live on average, 20 days longer than without.

Describe what is NOT a great client/referral for you

A referral that is still receiving aggressive treatment such as chemotherapy, dialysis, or doesn't have a POA. Not having a POA makes it difficult to have approval as swiftly but isn't impossible. There's more work involved but can be done.

What geographical area do you serve?

We serve all of the San Diego Area but I cover North County specifically.

We are a nationwide company and have Home Health and a Long Term Acute Care Facility further south.

What is the biggest misconception about your business?

There are several but the biggest is people don't know that it doesn't have to be during the last hours, days, or weeks of your life.

You don't have to "give up hope."

You don't have to stop seeing your physician.

There are no out of pocket costs.

What are great questions to ask our clients to find out whether they would be a great client for you?

Have you had a decline in your activities of daily living or have you found it more difficult to take care of yourself now than 6 months ago?

Have you had multiple infections? Admissions? ER/doctor visits?

What is the biggest misconception about your business?

Medicare or insurance sales

  • Medi-Cal Benefits is the Medicaid for California. It is a state program ran by each county. We do not sell insurance however we obtain needed medical benefits.

Free Service

  • Although we wish we can help everyone and for no cost. At this time, we charge a flat fee up front to cover time, services and knowledge.

“I don’t qualify”

  • Clients seem to think that they have too much income or assets to qualify. There are always ways to protect these assets and obtain benefits at the same time. This is where our services come in.

Medi-Cal will take my money or home

  • There is an Estate Recovery Program for those over 55 years of age and on Medi-Cal Benefits after death. We complete a plan to assure this will not happen to any assets you may have.
