Changes to how you order prescriptions

With effect from 1st November 2016 Bradford District Clinical Commissioning Group has asked that GP’s change the way they accept prescription requests and are no longe able to receive requests from the pharmacies.

Patients will need to order prescriptions directly from the GP practice.

There are a number of ways this can be done:-

·  In person at the practice using either the repeat script from the right hand side of your prescription or you can complete a repeat prescription slip and post it in the box in reception. A family member or friend can do this on your behave if you are unable to get to surgery.

·  You can register for our online service where you can order your repeat medication online. You can also use this service to make and cancel appointments. If you wish to regiser for this service please ask for a form at reception.

·  You can order your repeat medication via email at .

·  You can post your repeat medication request to the surgery.

We will no longer be able to accept prescription requests from pharmacies. This will not affect the collection and delivery service you use at your nominated pharmacy.

If you receive your medication via a dossett box, the pharmacy will still be able to order this on your behalf.

Ordering of prescriptions over the telephone

This service is only available for housebound patients.

Open Access Clinic

The practice is looking into running an open access clinic and we value feedback from our patients on how the service should run.

Should we

A)  Run a sit and wait clinic

B)  Ask patients to ring and book an appointment for that day.

Patient Led Groups

Art Therapy Class

The group is going very well and we have had some positive feedback from patients. We want to expand our creative corner to try and involve more patients who could benefit from the group. You do not need to be an artist to attend it’s a matter of getting out of the house meet new people have a coffee and a colour in.

The other groups we are trying to get off the ground are:

Knit and Natter group. If this is something you are interested in joining or helping to set up please get in touch.

Creative Writing Group. Nicky who runs the creative corner is a published writer and is happy to start a creative writing group. Again if this is something you are interested in please contact the surgery.

Gardening Group. If you are a keen gardener or a novice why not put your skills to good use here at the practice. It would be good to get some colour into the grounds. We would supply the refreshments to keep you going.

Walking Group

A patient has become a champion’s show the way volunteer and has set up a volunteer led walking group. It is run on a Monday morning at 10:50am lasting approximately 45 mins. Members meet near the entrance to the car park. Come along to improve fitness, enjoy some fresh air and meet new people.

Patient Self Care

Do you want to lose weight, feel healthier and get more active?

Perhaps you think you’re pinching an inch too many, your clothes are feeling tight or you’re worried about your health? A Better Life (ABL) can help you lose weight and lead a healthier lifestyle.

Over 12 weekly sessions, our experienced lifestyle coaches will guide you through personal goals, provide tailored exercise and offer advice around healthy eating to support you to lose weight.

If your whole family wishes to lose weight and improve their health together then maybe the Food, Activity, Balance (FAB) programme is for you. Over 12 weeks your family will be supported lo lose weight, eat healthier and get involved with more exercise. The programme is fun for all the family and children as young as 5 years old can get involved.

Both programmes are FREE so whether your goal is getting fitter for your summer holiday, helping you feel great in your clothes or perhaps you have a big family event coming up, we are waiting to hear from you and support you on your way to a healthier lifestyle.

Friday 19th May from 12pm-2pm we are having a patient self care event at the surgery. We will have representative from different organisation here to offer help and advice on different topics. Please come along and have a chat or just pick up some leaflets to take away and read at home.

The groups who have signed up for it so far are:-

A Diabetic Dietician will be coming to offer dietary advice to our diabetic patients. Our diabetic nurses will also be available.

Our Health Trainer Janet will be here to offer advice on weight management, healthy eating becoming more active etc.

Champions Show The Way –They can provide a range of interesting, sociable activities led by dedicated volunteer community health champions.

Social Prescriber –is a means of enabling GPs, nurses and other primary care professionals to refer people to a range of local, non-clinical services.

Alcohol Advisor – Arooje will be available to offer help and advice.

Creative Corner – Nicky who runs our creative corner group will be here to show you the different projects that the group are currently doing.
