Quay 430 CCTV Subject Access Request Form

The Quay 430 estate is covered by a Closed Circuit Television (CCTV) system. Images are recorded and retained for the purposes of health and safety and crime prevention. The scheme is operated by Rendall and Rittner on behalf of Quay 430 Management Limited.

The Data Protection Act provides Data Subjects (individuals to whom “personal data” relates) with a right to access data held about themselves, including data held on Closed Circuit Television (CCTV) systems. To enable Quay 430 Management Limited (or their agents) to deal promptly with your request for access, please complete this form, giving as much information as possible to help us identify your personal data. Under the terms of the Data Protection Act 1988, Quay 430 Management Limited has 40 days to comply with your request. The processing commences when your completed form and the fee of £10 (made payable to Quay 430 Management Limited) are received by the Data Protection Officer, although processing may be delayed if you have provided insufficient details.

Disclosure of personal data is limited to the Data Subject or authorised legal representative. To ensure personal data is not released to the wrong person, a Data Controller must be satisfied of the applicant’s identity. To that end the following information and documentation is required.

1)  A recent full-face colour photograph of the Data Subject.

2)  A document displaying the Data Subject’s home address, eg. A utility bill or bank statement.

3)  One additional piece of photo ID e.g. New style driving licence or passport (A colour photocopy will suffice in the first instance; the original document must be produced before viewing of data is permitted).

4)  For security reasons and to confirm identity, disclosure will take place in person at a location chosen by the Data Controller or their representatives. No copy will be supplied by post unless original photo ID is supplied with this application.
