As a participant in the Agency for Persons with Disabilities (APD) Consumer Directed Care Plus (CDC+) Program, you are required to remain current on program information and updated on program changes within CDC+.
Viewing this CDC+ Participant Refresher Training presentation will provide the participant and representative with important general and updated information on the CDC+ program. The terms “participant” and “representative” are used interchangeably. The full text Storyboard narration is available for reference.
Throughout the CDC+ Participant Refresher Training, you will actively participate in a Refresher Review at the end of each section. At the end of the presentation, you will take a Readiness Review. Answer the Readiness Review questions and complete the registration. The answers and registration information will be forwarded to CDC+ Central Office. The Readiness Review will be scored and retained as a record of training. Viewing this presentation, completing the Readiness Review and passing with 85% or better are program requirements. Failure to pass will result in a Readiness Review retake.
It is suggested that you have the CDC+ Participant Notebook and Appendix available for reference as you view this presentation.

Slide 6 / CDC+ HISTORY
CDC+ began in Florida in 2000 as an 8-year research and demonstration waiver called Consumer Directed Care (CDC), under the authority of an Independence Plus 1115 Waiver amendment granted by the Centers for Medicare and Medicaid Services (CMS). For the first 3 years, it was a strict research project that was measured and evaluated.
The 2002 Florida Legislature directed the Agency for Health Care Administration (AHCA), under the authority of Section 409.221, Florida Statutes, to seek a federal waiver amendment to implement the expansion of the Consumer Directed Care project.

Slide 7 / Data from the first 3 years of research showed that CDC participants reported greater satisfaction with supports and services, a higher number of achieved goals, and an ability to hire better employees among other important benefits.
As a result of the successful 3-year research project, in January 2004, the program was expanded and entered the 5-year demonstration phase, which was called Consumer Directed Care Plus (CDC+). During the demonstration years, all people who had been in the control group were eligible to participate.

Slide 8 / On March 1, 2008, CDC+ became a permanent Florida Medicaid State Plan option under subsection 1915(j) of the Social Security Act (SSA), as authorized by the Centers for Medicare and Medicaid Services.
Additionally, effective March 1, 2008, the Agency for Persons with Disabilities took over fiscal/payroll functions and customer service for the program, both of which had previously been performed by contracted private companies.

Slide 10 / CDC+ PROGRAM INFORMATION
CDC+ is a long-term care program alternative to the Developmental Disabilities / Home and Community Based Services (DD/HCBS) Medicaid Waiver.
CDC+ is based on the principles of self-determination along with person-centered planning that provides the opportunity to improve the participant’s quality of life.
CDC+ offers a framework that supports what is important right now, in the participant’s current stage of life and increases options for self-determination.
CDC+ participants are household employers. If the participant has selected a representative, then the representative acts on behalf of the participant in all areas of the program.

Slide 11 / CDC+ is a person-centered program that includes five (5) Principles of Self-determination:
1. FREEDOM to decide where and with whom the participant will live
2. AUTHORITY to decide how the participant will live their life
3. SUPPORT the participant will need to make decisions
4. CONTROL over the resources needed for the participant’s support
5. RESPONSIBILITY for the participant’s decisions and actions

Slide 12 / CDC+ is participant driven. This means the participant is expected to be involved in all aspects of the planning for their supports and services.
A monthly budget is deposited each month into a CDC+ account that the Fiscal Employer/Agent (FE/A) maintains for the participant.

WHEN supports/services will be provided and the quantity
WHO will provide the supports/services

Slide 14 / The CDC+ Program allows participants to choose their providers and employ who they want to provide the supports and services.
Submitting claims for payment is a very important part of CDC+ Program. Providers expect to be paid in a timely manner for the services they have provided.
Properly managing the Monthly CDC+ Budget is a critical requirement. Mismanagement of the monthly budget could lead to disenrollment from the CDC+ Program.
Participants and representatives are required to follow CDC+ Program rules.

Slide 15 / CDC+ PROGRAM PURCHASING PLAN
The participant is the decision maker. The participant chooses the supports or services that best meets their needs and goals identified in their Support Plan.
The participant and/or representative develop the Purchasing Plan, which is a monthly plan of the support or services to be purchased.
The CDC+ consultant can give technical assistance and/or provide guidance to a participant in the development of the Purchasing Plan, but cannot write the plan.
The CDC+ consultant’s role is to review for accuracy, sign and submit the completed Purchasing Plan to the APD Area Office according to established timelines.
A.16 Slide 16 / For all Restricted Services, the participant must ensure that 92% of the units of measure that are approved in the Cost Plan are budgeted in the Purchasing Plan.
All approved supports and services on a Purchasing Plan must have a Service Code obtained from current approved Service Code Chart. For detailed explanation of restricted & unrestricted services, reference the Service Code Chart in Appendix I.
All One Time Expenditures and Short Term Expenditures must have a beginning and end date stated on the Purchasing Plan.

Slide 17 / The participant is the household employer. All decisions to hire, manage and terminate providers are the participant’s responsibility.
The participant is solely responsible for appropriate use of public money. The CDC+ Monthly Budget must be used to meet the long-term care needs identified on the participant’s Support Plan/Cost Plan.

Slide 19 / CDC+PARTICIPANT ROLES
The participant is the decision maker and can manage the CDC+ Program requirements themselves or choose a Representative to assist on their behalf.
The participant makes decisions on how best to meet the needs identified on their Support Plan / Cost Plan, then evaluate how well the goods and services are meeting their needs and goals.
The participant is responsible to develop the Purchasing Plan and to ensure it remains updated by submitting timely updates to reflect their Support Plan / Cost Plan.

Slide 20 / The participant must use the CDC+ Monthly Budget responsibly to meet the identified long-term care needs and goals.
The participant is the employer and considered a small household business according to the Internal Revenue Service (IRS).
The participant is the authorized signer or may have the representative sign on their behalf.
The participant’s responsibility is to negotiate rates, hire, manage, terminate providers and ensure background screening requirements are met unless a representative has been designated.

Slide 21 / It is the participant’s responsibility to sign provider timesheets and invoices then submit claim information on time to pay all providers.
The participant is required to maintain an organized file system of paperwork associated with their participation in the CDC+ Program.
It is the participant’s responsibility to reconcile their CDC+ monthly statement and immediately report all discrepancies to APD CDC+ fiscal staff.

Slide 22 / CDC+ REPRESENTATIVE ROLES
As an advocate, the CDC+ representative should be intimately involved in the participant’s life and should also support their wishes and needs by making the best choices for them.
The CDC+ representative cannot be paid or be a paid employee that provides any other service.
The CDC+ representative can be listed as an emergency back-up of a critical service as an unpaid natural support and must have a Level 2 Background Screening prior to being listed on the Purchasing Plan.

Slide 23 / CDC+ representatives must be at least 18 years of age.
The CDC+ representative is the authorized signer. Although their role is essentially the same as the participant, only the participant is considered the Employer of Record.
The representative is responsible for approving timesheets & invoices, submitting claims for payment and complying with established payroll deadlines.
CDC+ representatives cannot be an owner, co-owner, stockholder, president, vice president or in any way benefit from any business authorized to provide services to or for the participant.

B.6 Slide 24 / CDC+ CONSULTANT ROLES
The CDC+ consultant is responsible for the participant’s annual support planning process and Medicaid eligibility.
The CDC+ consultant provides information, support and technical assistance as needed for matters such as choosing supports and services to meet the participant’s needs and goal and in planning for their future needs.
The consultant will monitor the participant’s health, safety & welfare and immediately reports any instance of neglect, abuse or exploitation.

Slide 25 / CDC+ consultant is responsible for maintaining all documentation of home visits and monthly contacts, including a monthly review of the participant’s Cash Check Log and Consumer Statements.
The CDC+ consultant ensures the participant complies with all CDC+ Program requirements.
CDC+ consultant is responsible for working with the participant and representative to develop a Corrective Action Plan if needed and follow-up as required.

Slide 26 / AREA CDC+ LIAISON
The area liaison authorizes the participant’s monthly budget to ensure program compliance.
The area liaison has final approval of the participant’s Purchasing Plan and any other program forms.
The area liaison oversees local program operations, Corrective Action Plans and background screening.
The area liaison facilitates communication between the participants, consultants and the CDC+ Program Central Office.

Slide 27 / CDC+ CENTRAL OFFICE
CDC+ Central Office is located at the APD Central Office in Tallahassee.
The CDC+ Central Office, in coordination with the Agency for Health Care Administration (AHCA), develops all policies and procedures for the administration of the CDC+ program.
CDC+ Central Office administers the CDC+ Program to ensure that all state and federal required guidelines, policies and procedures are followed, which includes quality assurance and monitoring.
The Central Office provides customer service and on-going technical assistance to the participant, representative and consultant, by answering questions and providing explanations.

Slide 28 / CDC+ FISCAL / EMPLOYER AGENT
The CDC+ Fiscal / Employer Agent (F/EA) receives the participant’s monthly budget amount from Medicaid and maintains it in the participant’s account.
The F / EA roles include assigning CDC+ Provider ID numbers and providing banker, bill payer and tax payer services.
Monthly Statements are sent to the participant showing the amount of money that was deposited each month, the purchases that were made, and the ending balance of the participant’s account at the end of the statement month. It is the participant’s responsibility to reconcile their CDC+ statement monthly and immediately report all discrepancies to APD CDC+ fiscal staff.

Slide 29 / SUPPORT PLAN / COST PLAN
The Support Plan identifies the participant’s needs and goals.
The Cost Plan indicates the amount of funding that has been approved in the Developmental Disabilities / Home and Community Based Services (DD/HCBS) Waiver to meet the needs and goals. The participant’s CDC+ Monthly Budget is calculated from their current approved DD/HCBS Cost Plan.
Supports and services purchased must meet the identified needs and goals listed in the participant’s Support Plan.
A participant or representative must obtain a copy of the Support Plan / Cost Plan from their waiver support coordinator/consultant anytime there is an update to the Support Plan.
