  • I have attended the information meeting about CO-OP classes, read the syllabus for the class, and my name will appear on the class roster.
  • I thoroughly understand the course description and expectations of the CO-OP and work-based learning.
  • I will complete all of the pre-class paperwork and adhere to the due dates.

Student Agreement

I have read the description and syllabus for the CO-OP courses and am familiar with the course expectations and accept the requirements. I agree to turn in all pre-class paperwork, keep up with turning in weekly work records on time, not change employment during the semester, and working based on the schedule I have provided. Due to the specific deadlines, I understand that turning late work will be for recordkeeping purposes only. I understand that changing jobs, losing my job, or getting fired is cause for failure for co-op. I understand that my success in CO-OP is primarily my responsibility.

HONOR CODE

Individual student integrity is a standard of excellence for CO-OP students. All students are expected to complete their work records with their supervisor’s signatures and work according to their required job duties. Supervisor signatures must be genuine and authentic. Each student will follow the DPS policies and FHS procedures in dealing with any breach of academic integrity, otherwise may face possibly failure of course and disciplinary action.

COMMITMENT CODE:

Students wishing to participate in CO-OP are expected to make a commitment to complete the course for the semester. Each student will remain in the job position for which they have provided all completed paperwork. Per the Fordson High School Handbook, “Student schedules will only be changed if the schedule poses a conflict with a district and/or state graduation requirements. Schedules will not be changed for any student unless there is a pre-standing issue that forces the change. Schedules are prepared for students based on class offerings and times.” Schedule changes will also not occur to honor student preference or teacher.
