Deadline Dates for After School Snack Intake Form

All After School Snack Programs starting September 1, 2010 thru December 31, 2010 must be submitted August 30, 2010 thru October 30, 2010.

All After School Snack Programs starting January 4, 2011 thru February 28, 2011 must be submitted November 30, 2010 thru December 20, 2010.

All After School Snack Programs starting March 1, 2011 thru May 30, 2011 must be submitted February 1, 2011 thru March 1, 2011.

If snacks are needed for testing, the cafeteria manager should be contacted for the procedures.

After School Snack Intake Form

Step One is to complete the After School Snack Intake Form and fax it to Deborah Torres at (410) 396-8724. The start date must be 2 weeks after the approval date to allow time to order and receive snack items. Remember, if your school does not meet the required 50 % of school population of free and/or reduced you will be required to provide your budget account number. All programs must be at least 4 weeks in existence. All incomplete forms will be returned for completion. When the program has been approved the coordinator will receive a faxed confirmation for snacks. The program should not expect snacks until 2 weeks after receipt of approval. The cafeteria manager or person-in-charge will be notified to place order.

NOTE: If your program ends in December and starts back in January only complete one Intake Form for the complete time frame.

Accountability Sheet

The accountability sheet is part of the mandated reimbursement procedures. This sheet is used to record all students who are enrolled in the After School Program. Site Coordinators must write/type each student enrolled in the program on the accountability sheet and give to the cafeteria manager or person in charge. The manager or person in charge will be responsible for completing this form. All updated student enrollment must be giving to cafeteria manager or person in charge.

Saturday programs coordinators/site managers are responsible for completing the accountability sheet(s) and must submit a copy to the cafeteria manager or person-in-charge on the next school operation day.

Meal Accountability Report

The meal accountability report is the second part of the mandated reimbursement procedures that is used for all Saturday programs. This report will be issued by the cafeteria manager or person-in charge. The cafeteria manager or person-in-charge must complete Section I. Section II must be completed by the person receiving and issuing the snacks. The completed form must be returned to the cafeteria manager or person-in-charge on the next school operation day.

Menu

The after school snack menu is the third part of the mandated reimbursement procedures. The menu is provided to ensure that we are meeting the meal pattern requirements.

Distribution of Snacks

All snacks must be issued by a designated cafeteria staff within thirty minutes after the dismissal bell.

Finally, if there are any adjustments/changes made to the original start date, day or time of the program you must notify the Food and Nutrition Service office ASAP. On-site audits are required for each After School Snack Program by a Central Office employee within the first couple of weeks of operation. It is critical that the date and time of the program is accurate so that an audit can be scheduled in a timely fashion.

By signing below, the site coordinator agrees to the following:

·  The after school snack program will start two weeks after the intake form is completed, faxed and approval received.

·  Incomplete forms will be returned for completion which will delay the start date.

·  The person responsible for issuing the snacks must complete the accountability sheet daily and make sure each student takes a complete snack (both items).

·  The accountability sheet must be returned to the cafeteria manager daily.

·  Section I of the meal accountability report will be completed by the cafeteria manager and given to the after school staff/coordinator.

·  Section II of the meal accountability report will be completed by the person receiving and issuing the after school snacks.

·  The person completing Section II of the meal accountability report must return completed form to the cafeteria manager daily.

·  Notify ______ASAP of any/all changes made to the original start date, day or time of the after school snack program.

Further,

·  I understand that if at any time procedures are not followed; the program will be in non-compliance and subject to cancellation.

·  I understand that the after school snack program will be monitored randomly to ensure compliance with all federal regulations are being adhered to.
