RE: Fabulous 50 Awards

It’s time once again for the NCHCFA “Fabulous 50” awards! A nomination form is enclosed with this memorandum. This time honored event provides the opportunity to acknowledge front line unlicensed personnel who have demonstrated outstanding achievements in the long term care profession. The Fabulous 50 awards program honors individuals who are the backbone in our facilities because of the care they provide to residents. A panel of judges from the Education and Programs Committee will select ten nominees from each district. Each of the ten award recipients will be honored at a dinner and awards presentation in their district. Of the ten selected, one nurse aide per district will then be selected to receive the “Nurse Aide of the Year” award during the Annual Convention & Expo. New! In addition, one recipient in a non-nursing discipline will receive the “Riley W. Clapp Outstanding Service” award during the Annual Convention & Expo. The dates for the 14th Annual Fabulous 50 Celebrations are:

District I October 15, 2013 Grand Bohemian Hotel, Asheville

District II October 21, 2013 Hilton Charlotte University Place, Charlotte

District III October 24, 2013 Grandover Resort, Greensboro

District IV October 29, 2013 Mad Boar Restaurant, Wallace

District V November 4, 2013 Renaissance North Hills Hotel, Raleigh

You will be notified by phone if a nominee from your facility is chosen to receive one of the Fabulous 50 awards. The recipient will be invited to attend a dinner held at the district level. Each recipient will be able to invite up to 7 guests. It is important that at least one facility representative attend the dinner with the honoree. If an employee is selected, you will be asked to submit several photographs for use in a special awards presentation. Nominees must not be a previous Fabulous 50 winner, and have at least five years of experience in the long term care profession. Please submit only one staff member per discipline.

Please pay special attention to the deadlines. Nominations received in the Association office after Tuesday, September 3rd, 2013 will not be considered. Nominations will not be accepted without the appropriate forms. To complete nominations on-line or to obtain additional forms, visit the Member Center of the Association Web site at http://www.NursingHomesNC.com. Click on “News & Updates”, select “2014 Fabulous 50 Nomination Packet” and choose “Survey Monkey”. If you have any questions about the Fabulous 50 awards, call the Association office at (919) 782-3827. The specific requirements for the awards are outlined on the next page.

Criteria:

·  For judging purposes, DO NOT INCLUDE YOUR FACILITY NAME in either essay.

·  All forms must be TYPED.

·  Only one staff member per discipline may be nominated from a facility.

·  Nominee must not be a previous Fabulous 50 winner.

·  Only unlicensed personnel may be nominated.

·  Nominations must be submitted on the attached forms or electronically. Notebooks, etc will not be considered.

·  Nominee must have a minimum of 5 years of service in the long term care profession.

·  Essay from facility staff member describing why nominee should be a Fabulous 50 winner.

·  Essay from nominee about why they have chosen their profession.

·  Nomination forms must be submitted by September 3, 2013.

·  All Nurse Aide of the Year winners, the “Riley W. Clapp Outstanding Service” award recipient, and each facility administrator must be available to attend the Awards Banquet at the Annual Convention on Tuesday, February 25, 2014 in Greensboro, North Carolina.

The 2014 nomination form will include two essays.

ESSAY #1: Paint a picture to describe why your nominee should be a Fabulous 50 winner. The following information may be included in Essay #1:

·  How has the nominee demonstrated leadership among other staff?

·  How does the nominee go beyond normal duties to make the residents feel at home in the facility?

·  List interpersonal skills that the nominee possesses that are most admired by other staff. (i.e. sincerity, helpfulness, ability to listen, ability to relate to a variety of people, talking with family members, etc).

·  Does the nominee participate in activities outside of the facility that enhance their job performance? If so, please give an example.

·  Testimonials from residents, families and coworkers are encouraged!

ESSAY #2: Why is this my chosen profession?

(The nominee may write this essay or the staff member completing this form may assist based on information from the nominee)

XXXXX XXXXX Fabulous Fifties Nominee

XXXXX came to work for our facility 8/4/1997 as a Certified Nursing Assistant. She presented as a quiet, soft spoken young lady who quickly established close bonds with her residents. Then, as now, she maintains a positive attitude which is infectious with the other staff members. Always the consummate team player, XXXXX is always willing to provide assistance to other staff members and can be counted on to perform to a high standard even in chaotic situations. She provides an excellent example to her peers. XXXXX can be counted on to be flexible in staffing, never complaining about unexpected staffing changes and performs her duties without reservation. Her ability to adjust to changing conditions makes her an invaluable asset to XXXXX XXXXX. In addition, her contact with all residents has always been positive and loving. Always kind, XXXXX will help residents with make up, appropriate clothing and details that make the resident feel special. XXXXX’s interpersonal skills are many, her willingness to help others, her attitude and true compassion for the residents are assets that are admired by others. XXXXX’s brother Mark wrote a book about her called “XXXXX’s Song” depicting a graphic view of his sister’s growing up in the last days of apartheid in South Africa when townships became battlefields and liberation came before education. XXXXX’s dream was to become a nurse. With her brother’s help, she was able to come to the United States where she continues to pursue this dream. We are glad that XXXXX made us a part of her life. She has blessed both residents and staff with her humbleness and kindness. We believe she is very worthy of being selected as a Fabulous Fifties Winner. XXXXX XXXXX RN Director of Nursing How many times have you heard someone say,” She is the sweetest person that I have ever met?” Well, when I speak those words about XXXXX XXXXX it is for real. She has the quiet strength of Samson from the Bible and a smile that will melt anyone’s heart. One night I was sitting down at the nurses station charting and began to hear someone humming a very soothing tune, it was XXXXX. She is always smiling and singing while she is performing her duties. She has a smile that brightens the entire facility. If we all could catch some of the rays that her personality gives off it would be so bright that her XXXXX town in South Africa would know it was she who had brighten the day. When I learned that she had a book called XXXXX’s Song that was about her I wanted to read it. Once I started reading her memoirs’ I couldn’t put it down. Through all the many struggles and challenges that life brought upon her, she is still standing. I could not imagine all the struggles that she had to endure, yet she has the gentle touch when it comes to her caring for her residents. She likes to write poetry and is there to listen to you when you need someone to talk to. We often talk about our decorating ideas, but her true desire is to become a nurse. I have no doubt that one day she will be the best nurse in the world. William Feather said, “Success seems to be largely a matter of hanging on after others have let go.” XXXXX is without a doubt a true inspiration to all and I am glad to be her friend and coworker.
