THIS AGREEMENT, dated as of ______, by and amoung "Alan Murray dba

NYNOTARY4U.COM' whose address is PO box 633, Copiague, NY 11726, hereinafter referred to as

"NOTARY”, and ______, an Individual, hereinafter referred to as the "Client'; That

client’s name
1. Service/Duties.

(a) The Client and the NOTARY agree that my sole duty is to provide Notary Public Service at the Event. No other duties other than the one prescribed herein have been assigned to the NOTARY.
2. Compensation.

(a) The Client agrees to pay the NOTARY a minimum of Three Hundred Fifty Dollars ($350.00) for the first five (5) hours of service and for any part thereof.

(b) After the fifth (5th ) hours, the client agrees to pay for service at the rate of one Hundred Dollars ($100.00) per hour and for any part thereof.

3. Cancellation of Service.

(a) If the EVENT is cancel/rescheduled, with one day advance confirmed notice, the above compensation indicated in 2(a) and 2(b) will be refunded if not rescheduled. Confirmation of cancellation shall consist of live personal Voice contact with me. Email, fax, voicemail, text, twitter messages, or other electronic delivery are not acceptable. If notification is given with less than one day advance notification, it is up to The Notary if any funds paid should be refunded.

(b) If the EVENT is cancel/rescheduled without proper notification etc, the above compensation is still payable at a Fifty Percent (50%) reduction, which is One Hundred Fifty Dollars ($150.00).

3. Effective Date, Term And Termination.

(a) This agreement is for the time period starting at ___:___ _M on ______, 20___.

(b) If for any reason the EVENT is rescheduled to another date this agreement shall remain in effect for each subsequent date, provided both the NOTARY and the Client agrees to keep this contract in effect. Otherwise, this agreement shall terminate the following day on the first day following the EVENT.

(c) This contract may be terminated by the NOTARY for any reason at any time without "Cause"

4. Entire agreement.

(d) This Agreement contains the entire understanding and agreement between THE NOTARY and the Client. There are no other agreements, conditions or representations, oral or written, express or implied, with regard thereto. This Agreement may be amended only in writing signed by the NOTARY and the Client.

IN WITNESS WHEREOF, the NOTARY and the Client hereto have executed and delivered this Agreement to be effective as of the day and year provided herein.
