Pre-Participation Physicals 2017-2018

This year, all athletic paperwork will be completed online. This process will allow us to be more efficient in handling the forms, as well as save valuable resources.

You must have your athlete’s school ID number in order to complete the forms. The forms can be found by going to and clicking “fill out electronic forms” in the upper right hand corner. From there, follow the prompt and select “Pleasanton ISD”.

Once at Pleasanton’s home page, you will hover over the “electronic forms” tab, and select the form you wish to fill out. Enter in all of the information, sign by holding down your mouse while in the signature box, and click submit at the bottom of the page. You will be redirected to a page saying your form has been submitted for review. If you have not been redirected, you may have left a box blank. If something does not apply to your student, just type “NA” in the box. Repeat this process until you have completed every electronic form. Do not place the zero at the beginning of your child’s ID number.

If your student has a history of asthma, but will not bring an inhaler to use during practice, please fill out the inhaler notification found under the “download and print” tab.

If you plan on getting a yearly physical with your primary care physician, please use the UIL medical history form attached. The form can also be found under the “download and print” tab.

  • The annual Mass Physicals will be completed Monday, May 22nd, 2017 beginning at 4:00 pm in the Pleasanton Athletic Complex. The cost will be $15 Cash Only. Please bring the UIL Pre-Participation Physical Evaluation Form with the medical history portion completed.
  • During the mass physicals, there will be laptops availablefor your convenience to fill out the required participation forms. You must have your athlete’s school ID number available.
  • Pleasanton ISD requires annual physicals for ALL student-athletes; grades 7-12.
  • The UIL Medical History Form and Pre-Participation Physical Evaluation Form will be required. No other Physical Exam forms will be accepted.
  • Physicals dated prior to May 1, 2017will not be accepted for the 2017-2018 School Year.
  • All required PISD & UIL Paperwork must be on file with the athletic trainer prior to the student participating in any practice, scrimmage, workout or contest (to include Athletic Period).

These forms are required each year your son or daughter wishes to participate in athletics for PISD. The Athletic Department would like to thank you and your son/daughter for filling out the forms and submitting them in a timely manner.
