Project Description
Describe the Project.
Why do you need the equipment?
How will the equipment be used?
What modification to the standard setup must be made?
Have you conducted an experiment design?
Show your test matrix?
How long do you think this experimental will take to complete?
How will you post process the data?
What are you expected results?
Hazards
List any known hazards that the operator and lab community may be exposed to during the experiment. (Mechanical, Chemical, Thermal, etc.)
What will you do to mitigate the hazard?
What emergency equipment may be needed?
Do you have access to this equipment?
Where is it located?
Technical Risks
List the known technical issues and risks identified as obstacles to success.
What could go wrong in the experiment?
Sources of Operator Error
Sources of Calibration Error
Experiment Design Problems
Unusual Material Behavior during Testing
etc.

I ______have taken the required safety training for access to laboratory and have been trained to operate this equipment. I will follow the testing procedure in this document and I will not deviate from the proper operation of this equipment.

Operator Signature: ______

Pretest Requirements
Personal Protective Equipment (Safety glasses, lab coat, steel toe shoes, appropriate gloves)
Lab safety training (operator has valid lab safety training)
Testing approval (approved by the PI)
Previous machine operation training (aware of warning signs and relative remedy)
Emergency STOP switch. (know the switch and how to react)
No contact (all moving parts are secure and are not in contact with anything i.e. cover, wire etc, electric connections are set)
Possible risk assessment for the test (i.e. the specimen strength is well below the machine capacity, specimen behavior during test and at failure etc.) and corresponding safety measure. (must be approved by the authority)
Simulate the test method while the table frame is off.
If 3D DIC system is planned to use with INSTRON, please also follow the 3D DIC procedure.
Fill out the log sheet of this machine.
Specimen Setup
If it's your first test have someone experienced be you while setting up.
Make sure the grip holds good portion of the specimen.
Mount the loaded grips with the load cell by distance adjusting wheel not the button. Must avoid grip compression during adjusting otherwise the specimen and the grip may be damaged.
The extensometer sensor teeth is sensitive, do not rub it against the specimen.
Before test make sure to set the load and extension to ZERO from the software.
Testing Requirements
Crosschecked. (after setting the specimen and just before the test have another operator crosscheck your setup and the written test method in Bluehill3 software). Run the test.
Test Running sign. (secure the surrounding area and make labmates aware of running test).
Don't leave the machine during test unattended. (unless given permission by the PI)
Stay aware of all moving parts of the machine. Monitor the test progress.
Post Test Requirements
Secure the extensometer.
Dismount the sample.
Turn off the Machine.
If any special change is made in software or on the machine, please, undo it. Write note on the log sheet.
Inform the PI about the test competition.
Return this Document to the PI and indicate if revisions are needed.
