Job Description

Job title: Patient Care Co-ordinator

Responsible to: Senior Patient Care Co-ordinator and Practice Manager

Core Responsibilities

To provide a point of contact for patients and relatives. To act as a focal point of communication between patients, doctors and other members of staff. It is essential that Reception staff always adhere to the need for strict confidentiality.

Patient Care

  • Dealing with patients, visitors and other health professionals either in person or on the telephone.
  • Working with the computerised appointment system.
  • Taking details of visits requested by patients who are unable to attend surgery and allocating them to the doctors.
  • Dealing with patients enquires regarding results of tests.

Administrative Responsibilities

  • Ensuring correspondence, reports and records are filed in correct notes
  • Scanning of referral letters, medical reports etc directly into patients electronic notes.
  • Issuing repeat prescriptions, together with any recall letters.
  • Entering new patient registrations and updating patient information onto the database.
  • Processing patients’ records when they register and leave the Practice.
  • Processing of internal messages- practice notes.
  • Work closely with the GPs, Practice Manager and other members of the Practice Team to plan, develop and implement changes within the practice.
  • Other tasks as appropriate and directed by the Senior Patient Care Co-ordinator or Practice Manager.
  • Basic QOF coding/Breast screening and bowel screening normal results

Team working

  • Develop, apply and disseminate to all reception team own areas of knowledge and expertise
  • Continually seek ways of improving procedures and workflows to maximise effectiveness of reception team
  • Understanding of requirement of confidentiality at all times

Additional Responsibilities

  • Adding details of immunisations to the new patient computer record

Other duties

  • In keeping with the role of patient care coordinater and as may determined by the Senior Patient Care Co-ordinator or Practice Manager.
  • The practice places great emphasis on team working and aims to foster good relationships throughout the team. All staff are encouraged to attend appropriate staff meetings and take part in other activities important to practice life.
  • The changing nature of primary care means that any year may bring new challenges to take on. This means that the post holder should be prepared, as with all members of the primary care team, to take on appropriate tasks as directed by the Senior Patient Care Co-ordinator, practice manager and partners
