6.3Recording andreporting ofaccidentsand incidents

Policystatement

WefollowtheguidelinesoftheReportingof Injuries,Diseasesand DangerousOccurrencesRegulations (RIDDOR)for thereportingofaccidents and incidents. Child protectionmattersorbehaviouralincidents between childrenarenot regarded as incidents and there are separateproceduresforthis.

Procedures

Our incidentbook:

- iskept in asafeand secureplace;

- isaccessible to our staff and volunteers, whoall knowhowto completeit;and

- is reviewedatleast halftermlytoidentifyanypotential oractual hazards.

Reportingaccidentsandincidents

  • Ofstedisnotifiedassoon aspossible,butatleastwithin 14days,ofanyinstanceswhich involve:

- food poisoningaffectingtwo ormorechildren lookedafteronourpremises;

-a seriousaccident or injuryto, orseriousillnessof,achild inour careandtheaction wetake in response; and

- thedeath ofachild inourcare.

  • Local child protectionagencies areinformedof anyserious accident or injuryto achild,or thedeath ofany child,while inourcareandwe actonanyadvice givenbythose agencies.
  • Anyfoodpoisoningaffecting two or more childrenoradultsonourpremisesisreportedtothelocal

Environmental Health Department.

  • Wemeet our legalrequirements inrespect of[thesafetyofouremployees/mysafety and thesafetyofmy employees]andthepublicbycomplyingwithRIDDOR.Wereport tothe Health and Safety Executive (HSE):

-Anywork-relatedaccidentleading toaninjurytoa member ofthepublic(child oradult),forwhich they aretakendirectly tohospitalfor treatment.

-Anywork-relatedaccidentleading toa specifiedinjurytoone of our employees.Specifiedinjuries includeinjuriessuchasfracturedbones, the lossofconsciousnessduetoaheadinjury,seriousburns oramputations.

-Anywork-relatedaccidentleading toan injuryto oneofour employeewhich resultsinthembeing sunableto work forsevenconsecutive days.All work-relatedinjuriesthat lead toone ofour employees beingincapacitatedfor threeor moredaysarerecorded in ouraccident book.

-Whenoneofour employeessuffersfromareportableoccupationaldisease or illnessas specifiedby theHSE.

- Anydeath,ofachild or adult, that occursinconnectionwitha work-relatedaccident.

-Anydangerousoccurrences. Thismaybeanevent that causesinjuryor fatalities or anevent that does notcauseanaccident,butcould have done;suchas agasleak.

-Informationfor reportingincidentstotheHealth andSafety Executive is provided in thePre-school Learning Alliance'sAccidentRecord publication.Anydangerousoccurrence isrecordedin our incident book (seebelow).

Incidentbook

  • Wehave readyaccess to telephonenumbersforemergencyservices,including thelocal police.Where we areresponsiblefor thepremiseswe have contact numbersfor thegasandelectricityemergencyservices, and acarpenterandplumber.
  • Weensurethat ourstaff and volunteerscarryout all health and safety procedurestominimiseriskand that theyknowwhattodoinanemergency.
  • On discoveryofanincident,wereportit totheappropriateemergencyservices–fire, police,ambulance–if those services areneeded.
  • Ifanincident occursbefore anychildrenarrive,our managerriskassessesthissituationanddecidesifthe premisesaresafetoreceive children.Our manager maydecide toofferalimitedserviceor toclose the setting.
  • Whereanincident occurswhilstthechildrenare in our careand itisnecessarytoevacuatethe premises/area,wefollow theproceduresinour Fire Safetyand Emergency Evacuation Policyor,when on anouting, theproceduresidentifiedintheriskassessment fortheouting.
  • Ifa crimemayhave been committed,weaskalladultswitness totheincident make awitnessstatement includingthedateandtime of theincident,what theysaworheard, whattheydid aboutit andtheirfull name andsignature.
  • Wekeepanincident bookfor recording major incidents,includingsomeof thosethat that arereportable to theHealthand Safety Executive asabove.
  • Theseincidentsinclude:

- a breakin,burglary,or theft ofpersonalorour setting'sproperty;

- anintruder gainingunauthorisedaccesstoourpremises;

- afire, flood,gas leakorelectricalfailure;

- anattackonanadultor childonour premises or nearby;

- anyracistincident involving familiesorour staffon thesetting'spremises;

-a notifiable diseaseor illness,oranoutbreakof food poisoningaffecting two ormorechildren looked afteronour premises;

- thedeath ofachild or adult;and

- aterroristattack, orthreatofone.

  • Intheincident bookwe recordthedate andtimeoftheincident,natureof theevent, whowas affected, whatwas done aboutit orifitwas reportedtothepolice,and ifsoa crimenumber.Anyfollowup,or insurance claim made,isalso recorded.

  • Intheeventof aterroristattack,wefollowtheadvice oftheemergencyserviceswith regardtoevacuation, medical aid andcontactingchildren'sfamilies. OurstandardFire Safety and Emergency Evacuation Policy will befollowed. Theincidentisrecordedwhen the threat isaverted.
  • Intheunlikelyeventofachild dyingonour premises, theemergencyservices arecalledandtheadvice of these services arefollowed.
  • Theincidentbookisnot forrecordingissuesofconcern involvingachild. Thisis recorded inthechild's own file.

Common Inspection Framework

  • As required under the Common Inspection Framework, we maintain a summary record of all accidents, exclusions, children taken off roll, incidents of poor behaviour and discrimination, including racist incidents, and complaints and resolutions.
