An exciting opportunity has arisen for a Care Co-ordination Development Officer to work within Sparkle based at the Serennu Children’s Centre in Newport, South Wales. This unique, beautiful state of the art building, just off the M4, is home to an integrated day centre for children with disabilities/developmental difficulties between the ages of 0 and 18, providing health, social care and leisure opportunities in one location. It has been built in a joint initiative between the South Gwent Children’s Foundation charity (Sparkle appeal) and ABUHB and acts as a support network for the whole family unit with a wide range of facilities and leisure activities on offer.

This post has been developed by the Sparkle Appeal and is funded by The Big Lottery Fund. The service was developed following a consultation process with parents regarding the identified need for a sustainable, equitable and integrated care co-ordination service. The successful applicant will be based at the Serennu Children’s Centre and will be responsible for scoping the availability and benefits of care coordination within the Serennu and surrounding catchment area, working with all stakeholders, observing and assessing best practice used in other children’s centres and developing a workable long-term plan for more widespread implementation of care coordination.

The post holder will work alongside partner agencies to include the health board and social care to develop and support a pilot model of sustainable care co-ordination and will review and adapt the pilot as necessary to achieve a working, efficient integrated care coordination service throughout Serennu and the wider area.

The post holder will be committed, dynamic, innovative, extremely flexible and responsive to the needs of families. The development of this post will be shaped through close consultation with service users, South Gwent Children’s Foundation Trustees and partner agencies. Job Purpose

The post holder will be responsible for scoping the availability and benefits of care coordination within the Serennu and surrounding catchment area, working with all stakeholders, observing and assessing best practice used in other children’s centres and developing a workable long-term plan for more widespread implementation of care coordination.

The post holder will work alongside partner agencies to include the health board and social services to develop and support a pilot model of sustainable care co-ordination and will review and adapt the pilot as necessary to achieve a working, efficient integrated care coordination service throughout Serennu and the wider area.

Key duties

Undertake consultation with families, facilitating focus groups, developing case studies and digital stories and undertaking user evaluation to clearly understand families’ needs in respect of the support required to help them to co-ordinate their child’s care.

Liaise with relevant staff, and ensure that there is full understanding of the new role and how best to integrate it into teams working with families.

Clearly articulate the priorities for families and the outcomes they wish to achieve to ensure any new model of care co-ordination will achieve these outcomes.

Scope the remit of care co-ordination within Serennu, develop effective networks and maintain collaborative working partnerships with relevant partner agencies including those within social care, education, the third sector and health.

Provide administration support to the Care Co-ordination Forum, including preparing and circulating agendas/papers, note taking and follow through in respect of actions recorded at the meeting.

To actively promote Sparkle services to families and professionals both within and outside of the centre.

To promote, monitor and evaluate the effectiveness of the “Supporting Families is Everyone’s Business” model.

Research, observe and assess best practice used in other children centres and identify gaps in current services to inform a new model of care co-ordination at Serennu.

To examine eligibility criteria for care co-ordination services where they exist and to work with others in the development of criteria which meets the needs of children and families.

To work with other professionals to identify how parents could be better supported to be more empowered, building self esteem and confidence to enable them to manage with less support from services.

Develop a proposed new model of care co-ordination at Serennu, to include the development of a full business case and costings for discussion with partner agencies. Set clear quality indicators against which such a model can be comprehensively evaluated.

Once agreed, to lead the pilot of a new model of care co-ordination, review its effectiveness and refine and adapt the model, as appropriate.

Work alongside partner agencies to ensure the pilot is developed into a proposal for a core integrated care-co-ordination service within Serennu, delivered by health and/or social care.

Data collection and analysis to include assessing whether professionals are co-ordinating appointments for children, DNA rates, cancelled appointments by services, etc exploring the reasons and timeliness for these actions. Explore and trial strategies to minimise such events, ensuring feedback from trial models from service users and providers.

Support families by signposting them to the appropriate help and support offered from Sparkle and other providers at the centre, and where appropriate within the community.

Work with the Sector Hub Co-ordinators, attending CDT meetings where necessary, to understand how care co-ordination services can support the proposed new model of managed referral and assessment processes.

To undertake multi agency information events and days for professionals and parents to gain a common understanding of how care co-ordination can impact positively on the lives of children and their families.

To effectively communicate and disseminate the development work taking place throughout the duration of the role through social media, websites, email and any other appropriate means.

To be fully informed about the full range of services available to families attending Serennu to include appropriate signposting. To work closely with other members of the Sparkle team in supporting families.

To demonstrate a willingness to undertake training, development and learning opportunities to improve skills and knowledge in relation to care co-ordination itself and care co-ordination services.

To comply with the Health and Safety at Work Act 1974 procedures and processes.

To adhere to all relevant Sparkle and ABUHB policies and procedures.

To observe confidentiality in all aspects of work.

To volunteer for at least four Sparkle events, fun days, etc per annum.

To undertake any duties and/or times of work as may be reasonably required of you, commensurate with your grade or general level of responsibility within the organisation, at your place of work or based in any other establishment.

Special Conditions

The nature of the post will require a flexible approach to hours of work. There will be a requirement to attend meetings, run sessions and attend events etc, which take place at evenings and on weekends.
