Outstanding Faculty Awards 2014-2015

For the nominator: Nominators should ideallyinform the faculty member of their nomination by Tuesday, March 10th, which is two weeks before the deadline for the faculty member to submit materials, to ensure that the nominee has sufficient time to prepare his or her materials. You may nominate a faculty member after March 10th, but do make sure there is enough time for that faculty member to prepare their materials by the March 24th deadline. Extensions will not be available due to the short time the committee has to review applications.

The nominator should complete this nomination form, and give it to the faculty member who is being nominated so it can be turned in with the other required materials listed below on the due date of Tuesday, March 24th to SEC Room 205 by 12:00 p.m.

How do I know if someone has already won the award I’m thinking of nominating him or her for? Please note that if you are unsure of whether a faculty member has won an award when you are considering putting forward a nomination, you can check the list of previous award winners that will be distributed to each department and also posted on the COE website.

For the Nominee: Nominees for each of the awards should submit the following by the deadline of 12:00 p.m., Tuesday, March 24th, 2015. All materials (other than books submitted for the scholarship award) should fit into a 1-inch binder. See the awards brochure for the criteria for each award.

Scholarship

a. Completed nomination form

b. Curriculum vita

c. Maximum of three representative publications

d. One letter of recommendation describing the nominee’s accomplishments in scholarship. This can be written by the nominator (in addition to the nomination form), or another individual.

Teaching

a. Completed nomination form

b. Curriculum vita

c. Statement of teaching philosophy, maximum 500 words long (two double-spaced pages)

d. Maximum of three representative artifacts (teaching materials, syllabi, etc.)

e. Recent (within the past 5 years) instructor evaluations collected with the standard UNI form. If not available, UNI teaching evaluations from within the past 10 years should be submitted.

f. One letter of recommendation describing the nominee’s accomplishments in teaching. This can be written by the nominator (in addition to the nomination form), or another individual.

Service

a. Completed nomination form

b. Curriculum vita

c. Maximum of three representative artifacts (final reports, conference programs, etc.)

d. One letter of recommendation describing the nominee’s accomplishments in service. This can be written by the nominator (in addition to the nomination form), or another individual.
