Job Title:Support Coordinator SW Hertfordshire

Hours:30hours per week, spread over four/five working days, in agreement with the CEO. This post will cover three local authority areas – Dacorum, Watford and Three Rivers

Responsible To:Community Services Manager (CSM)

Base:Foundation House, Hatfield

Main Purpose

To support people affected by brain injury, and their carers, to lead as independent, worry-free and fulfilled lives as possible.

Tasks & Responsibilities

  1. To provide telephone and email information, advice and supported signposting to other agencies, as required.
  2. To provide clients and carers with self-help materials and opportunities to undertake training to help them understand brain injuries, their affects and how to manage them.
  3. To visit clients at home, undertake assessments and agree a client and/or carer support plans, where goals are identified and recorded.
  4. To support clients and or carers to achieve their goals and regularly monitor progress against them.
  5. To support/run ourcommunity-based rehabilitation group in Watford.
  6. To run peer support groups in three Boroughs anda carers support group and a brain tumour support group in SW Herts.
  7. Recruit, train and support a small local team of volunteers who will set up social activities for clients and carers.
  8. To refer clients and carers to our counselling service as required.
  9. To refer clients to our Occupational Therapists and our vocational rehabilitation programme, which helps clients get back to work, training, volunteering or other meaningful activities.
  10. To provide training to clients and carers on a variety of subjects related to brain injury and rehabilitation.
  11. To provide supported access to benefits advice and employment rights advice.
  12. To promote Headway Hertfordshire positively and provide the CSM with monthly reports, statistics against Key Performance Indicators, outcomes and case studies demonstrating the value and outcomes of your work for individuals.
  13. To undertake risk assessments for activities in accordance with our policies and procedures.
  14. To work in accordance with all Headway Hertfordshire’s policies and procedures.
  15. To develop links and working relationships with local neurology services, social care teams, NHS primary and secondary care teams, mental health teams and local voluntary groups.
  16. To manage a caseload of clients and carers and to keep accurate, legible and complete records of all contacts and ensure our database is maintained up to date.
  17. To ensure own professional development by keeping up-to-date with legislation, internal and external practices, policies and procedures relevant to role through reading and by, with the CSM, identifying and undertaking relevant training and participating in regular appraisals.
  18. Any other duties appropriate to assisting colleagues in the smooth running of HH as requested by the CEO or CSM.
