LIVE ACTIVE INCLUSION AND EXCLUSION CRITERIA
Inclusion Criteria
Referred patients must be: -
- Inactive, this includes patients accumulating 30 minutes of moderate intensity activity on most days of the week i.e. brisk walking, heavy housework or gardening etc.
- Over 16 years old
- Motivated to increase their levels of physical activity/address weight management
- BP < 160/90 (BP >=160/90 will be accepted if being monitored / treated)
“Patients WITHOUT established heart disease” referrals: - Patients with NO known established heart disease can be referred directly to Live Active Advisor.
“Patients WITH established heart disease” referrals: - For patients with established heart disease an ETT or further investigation by the Live Active Medical Screener or a cardiologist may be required prior to referral to the Live Active Advisor. Please send all completed forms to ETT site or Live Active Advisor (see referral form for details) except for patients with:-
- HOCM (Obstructive Cardiomyopathy) – refer to cardiologist *
- ICD (Implantable Convertible Defibrillator) – refer to consultant who did implant for advice *
- Symptomatic Aortic Valve Disease – refer to/consult cardiology for advice *
- If patient unable to walk on a treadmill please follow the guidance on the referral form.
*If cleared for exercise please send all relevant information and completed referral form to the Live Active Advisor who will liaise with Live Active Medical screener.
Exclusion Criteria
The following will not be accepted onto the scheme: -
- Patients who are REGULARLY ACTIVE, this is defined as patients doing 2-3 exercise or sports sessions a week i.e. swimming, gym, exercise class, badminton etc.
- Patients looking for discounted price for activities
- Patients unable to follow an individual or class based activity programme (severe cognitive or physical impairment)
- Patients who are NOT motivated to increase their physical activity levels
- Patients with a systolic BP greater than or equal to 180 mmHg
- Patients with a diastolic BP greater than or equal to 100 mmHg
- Patients with symptomatic aortic valve disease, HOCM, ICD - refer to/consult cardiology for advice
- Patients with, unstable angina, or aortic stenosis
- Patients with acute heart failure or CHF (NYHA 3 or 4)
- Patients with, uncontrolled tachycardia or uncontrolled arrhythmia or a resting heart rate100bpm
- Patients with poorly controlled diabetes; symptomatic hypotension; febrile illness or a new cardiac event in the last 8 weeks
Re-referrals
Patients cannot be routinely re-referred to the scheme.
Re-referral will only be accepted if the patient has experienced:-
- Unforeseen personal difficulties i.e. family bereavement etc.
- A new medical condition/difficulty i.e. broken leg or MI etc.
Patients who have simply lapsed from their lifestyle change should be directed to the goals they completed with the Live Active Advisor at their last consultation. A copy of this should have been sent to you by the advisor. These goals will help the patient re-introduce physical activity back into their life.
