ACKNOWLEDGE RECEIPT OF THIS ADDENDUM BY INCLUDING THE DOCUMENT ALONG WITH YOUR WRITTEN RESPONSE. FAILURE TO DO SO MAY SUBJECT PROPOSERS TO DISQUALIFICATION. THIS ADDENDUM AND ANY ATTACHMENTSFORMS A PART OF THE PROJECT DOCUMENT AND MODIFIES THEM AS FOLLOWS:

  1. Amend the RFP document to revisethe schedule for Site Visits, Submitting and Responding to questions, and Proposal Due Date. Notification regarding specific dates and times for site visits will be distributed separately.

Update RFP Scoring Matrix to include Community Benefit: Changes are highlighted.

Approach/Strategy / 30% / The firm’s ability to provide a plan to address the overall scope of work, based on Owner’s needs.
Technical Competency / 25% / The firm must demonstrate their ability to provide and clearly define a solution that addresses Owner’s needs. Consideration will be give to the type of equipment and cleaning processes offered in proposal.
MWBE Utilization / 5% / The firm must (1) note if it is a certified MWBE firm, (2) note if it has a certified MWBE partner, (3) describe its MWBE subcontractor participation plan, (4) describe its past performance in achieving MWBE participation goals or requirements.
Community Benefit Program / 5% / The responding firms willing to implement or support a Community Benefit Program shall provide (1) the name, type and detailed description of the program; (2) the expected duration of the responding firm’s involvement with the program, including whether the responding firm has current or past involvement with the program; (3) the manner in which the responding firm intends to implement or support the program; and (4) the quantifiable, expected results of the program.
Schedule / 15% / The firm shall provide schedule(s) addressing the requirements of each location. Include an outline of the task, frequency (day/week/month) of completing task, and the estimate of time in minutes/hours to accomplish each task. The record (via customer references) of the firm’s ability to meet established schedule and deadlines and to provide timely support and maintenance will be considered along with the firm’s current workload.
Cost / 20% / The firm shall, at a minimum, provide cost per facility and a total cost.

Add Appendix 9 for Lake Bowen and Lake Blalock are separate buildings. Will be send as an attachment. Approx sq footage is 1700 each.
