PLEASE READ CAREFULLY – Keep a copy of this form for your records!

  1. All photos must be sent electronically (CD –ROM, flash drive, or send to )
  2. All photos must be in compliance with Henry County dress code, including baby photos
  3. We will not accept copies of photos. If you need photos scanned, check with Staples, Office Depot, and/or the UPS store.
  4. Please scan at least 300 dpi to size; however, we prefer 600 dpi. Example: A 2x3 wallet will print as a 2x3 if scanned at 300 dpi. If it needs to be a 4x6 on the printed page you would need to scan this same image at 600 dpi.
  5. Please save your photos with your student’s name.
  6. Please send all digital photos, this form (signed), TYPED text, and payment in a plastic bag or envelope.
  7. Please do NOT send photos taken with a cell phone unless 100% certain they are of good quality and high resolution.
  8. We CANNOT remove the “proof” watermark from proofs (for copyright reasons) and we DO NOT edit photos.
  9. If you wish to use a portrait from Lifetouch that you purchased, let us know and we can get the image directly from Lifetouch.
  10. ALL MATERIALS MUST BE TURNED IN AT THE SAME TIME (photos, form, text & down payments).
  11. ALL PHOTOS WILL BE RETURNED within three weeks.
  12. If MAJOR changes are needed, we will attempt to contact the ad purchaser.
  13. Senior Ad Proofs will be sent as a PDF to parents for approval. Ads must be approved by parents before publication.
