The Requestor, Contract firm(s), Construction Management or CAS and KCA Representative will hold a meeting prior to work starting to ensure the following criteria is met:
  • All approval signatures on the permit before work begins.
  • Demolition Permit will remain at the site the entire time demolition is performed.
  • This permit identifies equipment or facilities to be demolished that are part of construction and maintenance projects.
Demolition requirements: When the Demolition operation is complete, or when the permit expires, demolition permits shall be turned over to the construction manager or CAS and returned to KCA for file retention.
NOTE! Demolition permits are required to be “Posted” at job sites, along with contact names and numbers for the Project Manager, Construction Manager/CAS.

Kalamazoo Pfizer Demolition Permit Procedure

SCOPE

The scope of this policy is to provide direction for completion of the Demolition Permit Form prior to the demolition of any Pfizer Inc assets.

This requirement applies to any and all Project, Service, and Maintenance Contractors, Construction Management/CAS and or Kalamazoo Contractor Administrationrepresentatives planning to demolish any Pfizer owned or leased property or on any Pfizer-directed project.

The Demolition Permit shall be submitted to the Contractor’s Kalamazoo Contractor Administration Representative who coordinates activities with the appropriate personnel for review prior to beginning any demolition operation.

The permit will identify equipment or facilities to be demolished. When the demolition is complete, the Permit must be returned to KCA for file retention.

PURPOSE

The purpose of developing a Demolition Permit is to review and document all aspects of a demolition project which will create a safe work environment and guard against injuries to contractors, Pfizer personnel, and property. All approvals must be obtained on the permit before work begins and the Demolition Permit will be posted and remain on the site for the duration of the project.

DEFINITIONS

Contractor- An independent company, their employees, and subcontractors performing maintenance, construction, demolition or other work/ services for Pfizer on a contract basis.

Contractor Competent Person –MIOSHA defined as the person who is capable of identifying existing and predictable hazards in the surroundings, working conditions which are unsanitary, hazardous, or dangerous to employees, and who has authorization to take prompt corrective measures to eliminate them.

Pfizer Construction Manager (CM) Representative- Superintendent or other individual assigned tomanage, oversee, or coordinate the Contractor who is performing the demolition.

Pfizer ContractAdministration Specialist (CAS) -Superintendent or other individual assigned tomanage, oversee, or coordinate the Contractor who is performing the demolition.

PfizerKalamazoo Contractor Administration Representative (KCA)- Pfizer Contractor safety representative responsible for safety and EHS oversight of the Demolition project.

Pfizer Project Manager –Pfizer colleague assigned tomanage, oversee, or coordinate the demolition project.

DEMOLITION PERMIT COMPONENTS

The Demolition Permit contains seven (7) components for which data must be entered. Those components are as follows:

  • Provide the name(s) of the company who is in charge of the demolition
  • Provide the name of the OSHA Competent person, as defined above
  1. Demolished Equipment– to be competed by operating owning unit
  • API Electronic Change Control
  • Walk down to identify items to be demolished
  • End calibrations for out of service equipment have been completed
  • Maintenance records have been updated

PROCEDURE

The Pfizer Construction Manager Representative assigned to the demolition project will complete the Demolition Permit and submit it to the Kalamazoo Contractor Management person responsible for the building to be demolished. KCA representatives are as follows:

The KCA representative will review all information provided in the permit. The KCA Representative will schedule a meeting to include the CM Representative, owners/representatives from the building to be demolished, representatives of the contract firm to do the demolition work, and Pfizer EHS representatives to discuss all aspects of the project. Upon successful resolution of al issues involved with the permitting procedure, the KCA representative will sign the permit.

The permit is to be displayed on the job site at all times.

The KCA rep will maintain a copy of the permit as well.

The Demolition Permit process is now complete.
