AGREEMENT
By enrolling in the University Induction Clear Credential Program, I, the student, agree to the above planned course of study and to the CTC renewal requirements listed on my credential. I also agree to the following:
· It is my responsibility to enroll in courses by the enrollment deadlines listed above
· Orientation must be completed before enrolling in MSSS Inquiry II and MSSS Fieldwork Support II
· I must purchase Tk20 to participate/manage my academic activities throughout the program
· Portfolio Review is completed during the final quarter of the program
· Courses must be passed with a grade of C or above; courses graded C- or below must be retaken
· A cumulative GPA of 3.0 or higher is required for the clear credential recommendation
· I must enroll in all first quarter courses before I may receive the $25 enrollment verification letter for the CTC

Choosing credit status:
· Orientation is a non-graded course; therefore, choose Do Not Record as the credit status
· For Fieldwork Support, choose P/NP
· For all other courses, choose letter grade
