APPLICATION FORM – MEDICAL STUDENTS

Only one car-sharing permit is issued per car-sharing group, and therefore should display ALL registration numbers required by the group members. There must be a minimum of 3 students in the car sharing group.

It is the responsibility of each car-sharing group to ensure that the valid permit is displayed whenever a car is parked on site.

Please see separate Conditions and Guidelines for additional details. All parking permits are issued in conjunction with the Staff Car Park Policy, and all permit holders all bound by its terms and conditions.

CHARGES

By entering into a car-sharing agreement, I agree to pay the charge of £11.25 per month (minimum payment) per car-sharing group.

Cheques should be made payable to Heart Of England Foundation Trust, and must represent the full payment for the duration of the placement at Good Hope. Cheques should be returned to the Estates Department along with the completed application form.

CAR SHARING CONDITIONS

MEDICAL STUDENTS

1.  Car-sharing permits will be stamped C.S. to indicate to the parking attendants that the vehicle is part of a car-sharing group.

All car-sharing permits must be displayed as normal in the windscreen of the vehicle. Permits must be visible at all times. Permits must not be defaced in any way. Any alterations must be made by the Estates Department during office hours.

2.  Each car-sharing group should identify one individual who will be responsible for ensuring that payment is made, and that the permit is valid at all times whilst on-site. This designated person should ensure that the application form is fully completed and returned to the Estates Department along with the correct payment.

3.  Each group must agree to make their own permanent sharing arrangements.

4.  Each group must agree to return the car-sharing permit if all members cease to permanently car-share at any time.

5.  Each group must inform the Estates Department if there is to be any interruption of greater than four weeks to car-sharing arrangements.

6.  All cars must be taxed and have a valid MOT, and all drivers must be fully insured. Provided that costs are shared and that no profit is made, most insurance policies are unaffected by car-sharing schemes, but we suggest that you check with your insurers.

7.  The use of car-sharing permits will be monitored, and any abuse (e.g. cars displaying a car-sharing permit which are regularly used by only one member of staff) will result in the withdrawal of the car-sharing permit. Notice will be issued that the permit is considered invalid, and continued use would result in warnings being issued and the possibility of clamping.

8.  Staff not eligible to form a car-sharing group are those staff who work regular hours (typically between 8.00 a.m. and 6.00 p.m.) and who reside within a nominal half-mile walking distance of the Hospital and routes served by bus services from Falcon Lodge and Walmley.

9.  Participation in the car-sharing scheme is on the understanding that Good Hope Hospital shall not be liable for any resulting loss, damage or personal injury.

10.  There must be a minimum of 3 students per car sharing group.

These conditions act as a supplement to the Good Hope Hospital Staff Car Park Policy, a copy of which is available for reference purposes at the Estates Department.

CAR PARKING RESTRICTIONS

(Information relating to medical students)

Introduction

Due to limited car park capacity, Good Hope Hospital is unable to provide all staff with the opportunity to park on-site.

The Trust has developed a Green Transport Plan and a Staff Car Park Policy to address this issue, and encourages all staff to consider alternatives to single-occupancy car use wherever possible. We encourage and promote alternative transport modes which reduce the impact of car use on our site and the environment, such as:

·  public transport

·  car-sharing

·  cycling

·  walking

(A leaflet explaining bus and train options is available from the Undergraduate Co-ordinate).

The Trust appreciates that due to shift patterns and our ever-increasing recruitment catchment area, public transport is not a practical alternative for a large number of staff. Unfortunately, we do not have sufficient car parking capacity to allow everyone who wishes to park on-site to do so, and therefore on-site parking for staff is by permit access only, for which a charge is made.

We also impose certain restrictions on staff:

Permanent Restriction

New staff who live locally (within a set restricted area) and who contractually work between the hours of Monday to Friday 8.00 a.m. and 6.00 p.m. are not eligible to use the on-site car parks unless:

·  they are a disabled blue-badge holder;

·  they have compassionate grounds or a medical condition that prevents the use of alternative methods of transport.

NB Any concession on these grounds is not indefinite, and will be subject to review.

Temporary Restriction

At present no on-site parking can be offered for new staff between our peak demand hours of 9.00 a.m. and 4.00 p.m. on weekdays, and a waiting list for parking permits is in operation.

Arrangements for staff who want to park outside those hours, or who work permanent nights when there is less demand on our parking areas, are available.

Car park usage is constantly monitored and it is our intention to release daytime parking permits as congestion allows. However, ongoing building works and conditions imposed upon the Trust by the city council are resulting in the loss of on-site parking spaces.

Medical Students

We encourage medical students to car-share. Students who agree to car-share (minimum three students per group) must travel together and may only bring one car on-site per car-sharing group. As car-sharers assist the Trust by making more efficient use of vehicles, which in turn places less demand on our limited car parking resources, there is no waiting list for the issue of car-sharing permits.

Charges

Car-sharing permits cost £11.25 per month (or part thereof) per car-sharing group. Cheques representing the full payment for the duration of the placement at Good Hope must be made payable to Heart of England Foundation Trust and must be presented at the time of application.

Off-Site Parking

Staff who intend to travel by car and park off-site are reminded that Good Hope exists as an integral part of the local community, and is keen to achieve and maintain excellent relationships with local residents, community groups, the City Council, the Police and other road users.

As a result, staff who choose to park in local residential roads are urged to demonstrate consideration and courtesy for residents, pedestrians and other road users. Good Hope cannot and will not condone inconsiderate, illegal or dangerous parking by any member of staff.

CAR SHARING GUIDELINES

·  Trial Period

We suggest a 6 week trial period, during which time everyone involved can review their arrangements if they don’t appear to be working well. The Transport Co-Ordinator will attempt to contact the individual members of each car-sharing group at the end of this period to determine whether all parties wish to continue with arrangements. No one should feel that they have to continue in a car-sharing group if they are uncomfortable doing so. Anyone who wishes to withdraw from a group but stay within the car-sharing scheme may do so.

·  Communication

Make sure that everyone in your car-sharing group has the ability to contact each other quickly. We suggest making work and home phone numbers available to all members of your group.

Agree on a procedure for notification. This allows you to change arrangements if the designated driver on a particular day falls ill, and enables everyone in the group to make alternative arrangements (e.g. someone else drives in that day). It may also be worthwhile agreeing a “reserve” driver who takes over if something goes wrong.

·  Route & Times

Establish the morning pick-up points and also the time and meeting place for the homeward trip.

·  Waiting Times

Decide how long the driver is expected to wait for passengers to arrive.

·  Ground Rules

We suggest that you discuss the following in advance to give your car-sharing group a better chance of succeeding:

o  Volume/style of music/radio stations

o  Smoking or non-smoking

o  Temperature

o  Eating

o  Whether or not you want to chat during the journey

·  Safety

Your safety is of paramount importance, so you need to be happy with the way that your fellow car-sharers drive.

Drivers should remind passengers that the law requires seat belts to be worn in the front and rear of cars. (For passengers under 14 years of age, it is the drivers responsibility to ensure that they wear seat belts).

·  Schedule of Drivers

If you are going to share the driving, decide whether you will alternate daily, weekly, monthly etc.

·  Reimbursement of Costs

If everyone shares the driving equally, this won’t be an issue. If not, you may wish to agree in advance the amount, regularity, and dates of payment.
