Steps for Creating Individual Accommodation Logs in Microsoft Word

1. Open PlanMaker

2. Click on the ESE Services Tab

3. Choose the export info button at the bottom of the page

4. Choose the Excel format to export the student services information

5. Close FileMaker Pro and go to your desktop. There will be an icon on your desktop that says “studentservices.xlsx”

6. Open the Excel document and scroll to the right until you see the “purpose” column.

7. Using the Student Date of Birth column (column D), you can determine how many of the rows in column J (purpose) go with each individual student.

8. Select the accommodations that the student receives by putting your cursor on the first accommodation you want selected (additional breaks in this case), hold the shift key down and use the down arrow to select all of the accommodations the student receives, once everything is highlighted go up to Edit and select copy or hold down the command button and the “c” button to copy the highlighted selection.

9. Open up the Blank Accommodation Individual Chart

10. Put your cursor right under the word “Accommodations” and paste the individual student’s accommodations. The document should be set up that the accommodations will be placed into two columns to keep everything on one page.

11. After pasting the accommodations on BOTH pages of the document, choose the numbering button to add numbers next to the accommodations (both pages). You may have to click the lightening bolt after hitting the numbering button and choose to “restart numbering” if it pops in starting at #32.

12. Type in the student’s name at the top of both pages after accommodations with numbers have been inserted on both pages.

13. Save the document using the last name and first initial of the student to keep for you to document their accommodations AND to email to every basic education teacher AND ESE teacher the student has so they too can document when they provide the student’s accommodations. (I would recommend that you create a folder on your desktop labeled Accommodation Logs 12-13 to store all of your individual Word documents.)
