TC-3 VETERAN ADMINISTRATION BENEFITS
VERIFICATON
Don’t forget to attach your business card or use your property address stamp.
This form should be used to verify the income from Veteran’s Administration for all household members. This form can also be used if you suspect are resident may be receiving VA benefits. These instructions should be followed:
1. Insure that the first half of the form is completely filled out with all pertinent information.
2. The person being verified must sign the verification form. This shows the VA that the recipient authorizes him/her to release the information.
3. Insure you have the correct names, claim numbers, file numbers, and social security numbers for the recipient being verified.
4. Make a photocopy of the signed original and place it in your pending file.
5. The original verification must be mailed to the appropriate VA office for completion.
a. If you do not receive a reply (within the specified business days) call the VA office and request the information over the telephone. Record the information you receive and write down the name and title of the person you spoke with as well as the date and your signature.
Verify the source’s mailing address and mail them a copy of the verification for their written confirmation. Direct them to sign and return for the file.
b. If VA office will not give out information, please call the Occupancy Specialist for assistance.
6. When the verification is returned to you, attach the envelope it came in to the verification form. Check the verification for completion. If the form is not filled out completely, comments are unclear, or amounts have been altered, you must call the VA office and verify the information in question. Be sure to record the name and title of the person you speak with, sign, and date it.
7. IN NO CASE IS A VERIFICATION TO BE HAND CARRIED TO THE VERIFYING PARTY BY AN APPLICANT OR RESIDENT.
