Originator: Name of the ENV Coordinator or the FMP if related to temporary updates.

Date of Submission: Self explanatory.

Permanent Change: Indicate whether it is a permanent or temporary change.

Effective From/To (date): If the change is temporary the effectiveness period will be no more than 2 days.

If the change is permanent the From date should be beyond the next AIRAC date.

Default Runway for: Self explanatory, use one form per Runway.

Default Taxitime Value: Self explanatory.

Temporary Taxitime Value: Self explanatory.

Temp. Taxitime Applicability: Weekly and/or daily pattern when the corresponding value is applicable.
