VIZSLA CLUB OF AMERICA
RESCUE ADOPTION APPLICATION FORM
This questionnaire is prepared for your benefit. Please take the time to fill it out as completely as you can. All of the information that you provide us with will be kept in strict confidence and will help us better place the right dog with the right family. An improper placement, in which all the details are not known, can end tragically, usually for the dog. Completion and submittal does not, however, guarantee that you will be eligible to adopt a Vizsla. Since Rescue operates totally on donations and adoption fees (a 501c3 organization),there is an adoption fee of $275 for dogs up to age 8-10 $100 from 8-10 and beyond that just a happy re home! Thank you for your interest in adopting a Vizsla.

***************Please realize that rescue is not an exact science and is undertaken by volunteers. When a rescue dog is in a foster situation and is ready to transition to a potential forever home, there may not be a long time line available. Our goal is to get rescue dogs into good homes quickly; to minimize stress on the animal.
If you are not able to take on a dog at the time the dog is ready to transition to its forever home, Vizsla Rescue may need to move onto the next available home. Foster families are not kennels or long term solutions.
*****IT IS VERY IMPORTANT THAT YOU STAY IN TOUCH WITH US, AS IMPORTANT AS US KEEPING IN TOUCH WITH YOU. IF YOU SHOULD GET A DOG BEFORE WE FIND ONE FOR YOU, PLEASE LET US KNOW SO WE CAN REMOVE YOUR NAME(S) OFF THE ACTIVE LIST TO MAKE ROOM FOR SOMEONE ELSE!!
******PLEASE ALSO RETAIN A COPY OF YOUR APPLICATION IN CASE THERE IS A COMPUTER GLITCH IN THE RESCUE PROGRAM AND IT GETS MISPLACED.
