PROCUREMENT OF PERMIT

Upon application to the school building principal, facilities will be made available whenever possible to all school affiliated groups and other organizations and persons meeting the following rules and regulations. By its signature on the front of this form Applicant-Organization agrees to be bound by and comply with each of these rules:

1.  The use of all school facilities for any purpose whatsoever shall be cleared through and initiated by the school principal. Upon completion of the application, the Applicant-Organization shall then present it to the Business Office for determination of fees, responsibilities and services required by the school system.

2.  The use of the building will be strictly confined to areas designated or included in the PERMIT. The Applicant-Organization will be held responsible for the observance of these rules by all persons participating in or pertaining to the activity. In case of doubt, the school custodian or other representative of the school system on duty shall have immediate authority in any matter covering the use of the building. Failure to comply with these rules and regulations will be sufficient reason to cancel future privileges of school facilities.

3.  All PERMITS issued by the School Board are subject to cancellation with or without due notice for any reason whatsoever.

4.  Gambling, drinking or the possession of intoxicants within the school building or on school property is specifically prohibited.

5.  No furniture or equipment (including pianos, stage equipment, audio-visual equipment, tables or chairs) shall be used or moved without express approval on the PERMIT or consent of the school principal.

6.  MCS and its employees shall not be responsible for damage to or loss of property upon school premises sustained by Applicant-Organization or any participant in a program or patron of any program held on school property. Applicant-Organization waives all rights of recovery against MCS and its employees for all losses, damages or injuries to any of Applicant-Organization’s property, or damage or injury to any person or participant affiliated with Applicant-Organization, during use of the described premises.

7.  The use of special equipment such as stage lighting, scenery, curtains, picture projectors, public address systems, folding goals, bleachers, etc., shall be permitted only when operated by school employees or other persons authorized by the School Board or its representatives.

8.  No signs, displays or materials may be attached to or mailed against the walls, window glass, woodwork, draperies, blinds, stage curtains, grounds, drives, etc., without expressed approval on the PERMIT or consent of the school principal.

9.  It shall be necessary for some organizations (depending upon the size of the group) to have police, firemen and parking lot attendants. These personnel shall be procured by MCS and charged to the Applicant-Organization. The concession rights are reserved for the school principal unless otherwise stipulated.

10.  Applicant-Organization agrees to carry at its expense a policy or policies of public liability insurance covering its use of the described MCS facilities. MCS shall be named as an additional insured therein, and the policy or policies shall have limits of liability of not less than one-million ($1,000,000.00) dollars for personal injury or death, two-hundred fifty thousand ($250,000.00) dollars for damage to personal property and ten-thousand ($10,000.00) dollars for medical coverage per person throughout use of the described facilities. Applicant-Organization shall deliver evidence of such coverage to MCS.

11.  Applicant-Organization agrees to indemnify and hold harmless MCS and its employees from damages caused by injury or death to any person or persons or damage to any property which occurs during Applicant-Organization’s use of the described premises.
