The VAWNY PGY2 Psychiatric Pharmacy Residency is a full-time, temporary appointment with an anticipated duration of one (1) year. Appointment is within the Federal government and residents are therefore subject to all regulations and requirements of the Federal system

  1. VAWNY PGY2 Pharmacy Residency Purpose Statement:

Graduates of this PGY2 psychiatric pharmacy residency program will have the knowledge and skills required to initiate new clinical psychiatric pharmacy services in any setting that cares for patients with neuropsychiatric illnesses, as well as the skills necessary to serve as a preceptor for pharmacy residents and pharmacy students.

  1. Qualifications:

PGY1 Pharmacy Residents must possess

  1. U.S. Citizenship
  2. Licensed pharmacist
  3. The resident must have completed an accredited PGY1 pharmacy residency program. The PGY2 Psychiatric Pharmacy Residency Program Director (RPD) will contact your PGY1 RPD in early June to ensure that the resident is on track to complete the PGY1 residency in time to begin the PGY2 psychiatric

pharmacy residency program in July.

  1. Resident Selection/Recruitment/Residency Promotion

TheresidencywillbepromotedthroughtheASHPWebsite/VAPublicWebsite/VAWNYPublicWebsiteaswellasvariousnationalmeetings

ResidentapplicantsarerequiredtoapplytotheprogramthroughPhorcas.Ascreeningrubricapprovedbytheresidencycommitteewillbeutilizedtoprescreenapplicantsforonsiteinterviews.Thescreening rubricwillbeapprovedbytheResidencyCommitteeandremainonfilewiththeProgramDirector.Thetop30(nottoexceed30interviews)scoredapplicantsinPhorcasWebAdmitwillbeselectedandcontacted foranonsiteinterview.TheRPDreservestherighttoextendtheapplicationdeadline,withapprovaloftheresidencycommittee,toservetheneedsoftheapplicationprogram.

Allonsiteinterviewswillbeconductedwithatleasttwomembersoftheresidencycommitteeandonecurrentresident(moreareencouraged).Theonsiteinterviewwillconsistofapresentationbythecandidate,interviewbyagroupoftheResidencyCommitteemembers,tourofthefacilitywithstopstomeetpreceptorsofeachrequiredrotationandwrap-upwiththeProgramDirector.Theinterviewquestionsandscoringgridwillbepre-approvedbytheresidencycommittee.Aftercompletionofthelastinterview,theresidencycommitteeandcurrentresidentswillmeetandconsolidateindividualrankingscoresbeforethefinalrankissubmittedtotheNationalMatchingService.Theresidencycommitteereservestherighttonotrankcandidatedeemedunsuitablefortheprogram. “Scramble”IntheeventofanunmatchedresidencypositiontheapplicantwillapplythroughPhorcasWebAdmit,asperthedatespostedontheNationalMatchingWebsite.TheRPD/ResidencyCommitteereservestherighttowaivetheonsiteinterviewandconductatelephoneinterviewofselectedcandidatesinlieuoftheinpersonvisit.PernationalVArequirements,allpositionsmustbefilledbyApril15oftherespective year,ortheymaybesubjecttotemporarytransfertoanothersite.

  1. Resident Acceptance:

Uponacceptanceintotheprogramtheresidentwillbeinformedinwritingofthetermsandconditionsoftheappointmentandprovideasignedcopyofacceptanceofsame,includingthecurrentPGY2-residentmanual.

  1. Resident Evaluation:

Theresidentwillbeperiodically(butatleastquarterly)evaluatedbytheresidencyprogramdirectorusingpersonalobservationsandcommentsfromtheotherpharmacistpreceptorsand/orhealthcareprofessionals.Observationsarebasedonadherencetoandcompletionofthegoalsandobjectivesasestablishedintheresidentoutlineandasdiscussedwiththeresidentduringroutineevaluations.

Necessaryimprovementswillbedocumentedandexpectedtoberesolvedwithin30days.Theresidentmayprintacopyofeachassessmentforhis/herfiles.Relativecommentsonsummativeorcustomizedplanswheretheresidentisinneedofimprovementtoachieveaspecificgoalwillbecommunicatedfrompreceptor(orRPD)tothenextrotationpreceptorverballyorinwriting.

Allevaluations(bothcompletedbytheresidentandpreceptor)shouldbecompletedusingPharmAcademicTMinatimelymanner,ideallypriortotheconclusionoftherotation.

Itistheresponsibilityoftheresidenttocompleterotationspecificevaluationsandself-evaluationson a monthly basis.

  7. DocumentationofhowthegoalsarebeingmetandhowtheprogramisbeingalteredtohelptheresidentachievethesegoalswillberecordedinPharmAcademicatthestartofresidencyandatleastquarterly.
  8. ThefollowingactionplanfortheresidenthasbeendevelopedbasedonthegoalsandrequirementsoftheIDPharmacyResidencyProgram. Itincorporatesanevaluationoftheresidentspastexperiences,strengthsandweaknesses,andtheresident’sindividualgoalsandinterests. AttheendoftheresidencytheResidentandRPDwillcompletethroughthecustomizedplan,theyearendevaluation.
  9. Ifself-assessment(reflectiveinquiry)isnotnotedtheRPDmayreturntheevaluationforimprovement.

(3)SP:ProgressAchievementrequiresskilldevelopmentduringmorethanonelearningexperience.Inthecurrentlearningexperiencetheresidenthasprogressedattherequiredratetoattainfullabilitybyendoftheprogram

(4)NI:Residentslevelofskillonthegoalorobjectivedoesnotmeetthepreceptor’sstandardsofeither‘Achieved’or‘satisfactoryprogress

  1. PharmAcademic – All residents will utilize

PharmAcademic:

UsingPharmAcademicwillaccomplishtheCustomizedResidencyPlan,Formativeevaluations,SummativeEvaluationsandPreceptorEvaluations.PrintingandplacingPharmAcademicdocumentsintheresidentbinderisoptional.

  1. Programassessment-Periodically,butnolessthanannuallytheResidencyCommittee withtheprogramdirectorwillreviewPharmAcademicProgramandPreceptorevaluationsandpresentanyfindingstothegroupfordiscussion/change.
  1. Preceptor,RotationandProgramEvaluation

Theresidentistoevaluateboththepharmacistpreceptors,non-pharmacistpreceptorsandtheindividualclinicalassignmentfollowingcompletionofhis/hertourofduty(butnotlessthanquarterly).TheevaluationwillbediscussedwiththeindividualpreceptorsandPharmacyServiceManagerasrequired.Theresidentwillreporttothepharmacyonadailybasis;coreandspecialtyrotationswillstemoutwardfromthepharmacy.

  1. Continuous professional (preceptor) development:
  2. Preceptors will individually develop CPD plan. This will include but is not limited to:

Demonstrationofatleast2hoursofannualcontinuingeducationthatisrelatedtotheircontentarea.Thiscanbeliveorwrittenaccreditedprogram,VAlearningopportunitiesorsimilarprogramsthatarerecognizedbyanationalcertification/accreditationbody.Preceptorswillroutinelypracticeintheirtrainingarea,demonstrateadesiretotrainresidentsandanexcellenceinteachingskills.Instructionwillbeofferedtofacilitatepreceptorspursuingthefourcoreareasofeducation:1)directinstruction,modeling,coachingandfacilitating.CPDandattainmentoftheASHP4/7preceptorrequirementswillbeaccessed onanannualbasisduringannualVAperformanceappraisal.

PreceptorsareencouragedtoparticipateinannualBCPSstudysessionsofferedbytheVAWesternNYHCS.

Annually,theRPDwillrequestallpreceptorscompleteaself-assessmentandreviewsameandpresentasummarytotheResidencyCommittee

Continuously,butnolessthanannuallyRPDwillreviewqualityofwrittenpreceptorfeedback.TheRPDwillreturnthroughPharmAcademic,incompleteevaluationsandpresentasummaryoftheside-byside reviews to theResidencyCommitteeaspartof annualprogramqualityassurance.TheRPD will beevaluatedaspartofVISN2OPPE(ongoingprofessionalpracticeevaluation).

  1. Attitude/Professionalism

Theresidentisexpectedtodemonstrateprofessionalresponsibility,dedication,motivation,andmaturitywithregardstoallactivitiesandresponsibilitiesassociatedwiththeresidencyforitsentirety.TheresidentshalldemonstratetheabilitytoworkandinteractwithallthestaffandpatientsoftheMedicalCenterina productiveandharmoniousmanner.Appropriateattire,personalhygieneandconductareexpectedatalltimes.TheresidentwilladheretoalltheregulationsgoverningtheoperationsoftheDepartmentofVeteransAffairsMedicalCenterwithoutexception.

  1. Human Resource Management Service Requirements:

You will be contacted by the Pharmacy Service Administrative Officer regarding documentation required by Human Resources Management Service (HR). A representative from HR will contact you with additional details including information on VA orientation, scheduling your physical and the start of your residency. You will need to complete all pre-employment items to include drug testing, fingerprinting, background checks and other items required for federal employment. There are multiple forms that will need to be filled out for pharmacy and HR and they will be provided on a separate occasion.

  1. Sexual Harassment and Discrimination Policies:

The VAWNY Healthcare System has a zero tolerance policy for any form of discrimination and sexual harassment. You will receive specific information on policies and procedures during your orientation.

  1. Attendance/Leave Termination:
  1. Attendance/Sick-leave/Holidays
  2. Allrequestsfortimeoffincludingvacationandholidaysmustbepre-approvedbytheRPD,rotationpreceptor,PharmacyServiceManagerordesigneeandfollowVAproceduresforrequestingleaveandbegivenwithadequatenotice(minimumof2weeks).Iftheseprocessesarenotfollowedtheabsencewillbeconsideredunexcused.
  3. Promptarrivalandattendanceisrequiredatallclinics,conferences,meetings,roundsandotherscheduledactivitiesduringeachandeveryrotationthroughoutthetermoftheresidency.
  4. Theresidentisresponsibleforreschedulingorarrangingalternatecoverageforallactivitieswhichwilloccurduringanyplannedabsence.
  5. Unexcusedabsencesandortardinesswillnotbetoleratedandcanbeabasisforfailureoftherotationinvolved.
  6. ItistheresponsibilityoftheresidenttocontactthepreceptororthepharmacyAOassoonasispracticaltoreportunavoidableabsencesortardiness.
  7. Weekendschedule-inordertomeetcurrenttimekeepingrules,theresidentwillhaveadayoffbeforeand followingtheweekendscheduled.Iftheresidentchoosestoworkthosedaysoff,thisisallowedbuthe/sheneedstosendtheVHABUFPharmacyTimekeepingamessagetotheeffectthathe/sheisworkingonascheduleddayoff.
  8. Extended Absence
  9. Intheeventofanextendedabsenceduringtheresidency,theRPDwillcoordinatewithHumanResources(andifneededtheVAOfficeofAcademicAffiliations-OAA)arrangementsneededtocompletetheresidency.
  10. Thetermoftheresidencyshallbeconsidered2080hoursandtheprogramwillnotbeconsideredcompleteuntilboththehoursandrequirementsaremet.
  11. ThePharmacyManagerwillbenotifiedofanyextendedabsenceduringtheresidency.
  1. Termination/Resignation
  2. Intheeventthattheresidentchoosesnottocontinuetheresidencyandmeetthestatedrequirements(hoursandlearningobjectives)heorshewillberecommendedforterminationbytheResidencyCommitteetotheChiefPharmacyService.Alternatively,theresidentcanchoosetosubmitaletterofresignation.
  3. Duty Hours
  4. To be in accordance with ASHP/ACGME standards the resident will supply regular communication detailing all extramural (moonlighting) hours worked. This will be documented on the customized resident plan with the residency program director, no less than quarterly.
  5. Maximum Hours of Work per Week
  6. Duty hours must be limited to 80 hours per week, averaged over a four-week period, inclusive of all in-house call activities and all moonlighting.
  7. Moonlighting (i.e., working outside the residency program) is highly discouraged.
  8. Moonlighting must not affect the resident’s judgment while on scheduled duty periods (as assessed by the preceptor or other supervising entity), interfere with their ability to provide safe patient care (as assessed by the preceptor or other supervising entity), or impair their ability to achieve the educational goals and objectives of their residency program (as assessed by the preceptor and/or Residency Program Director (RPD).
  9. Residents not meeting the requirements of their residency program as a result of moonlighting will be required to comply with a remediation plan outlined by the RPD and, if no improvement is seen, will be subject to dismissal from the residency program.
  10. All moonlighting hours must be counted towards the 80-hour maximum weekly hour limit.
  11. If residents moonlight, they must submit their hours to their RPD on a monthly basis. If the number of hours exceed the above limit when averaged over a four week period, the resident will be expected to reduce the number of hours they are committing to moonlighting so as to meet this requirement.
  12. Mandatory Time Free of Duty
  13. Residents must be scheduled for a minimum of one day free of duty every week (when averaged over four weeks). At-home call cannot be assigned on these free days.
  14. Maximum Duty Period Length
  15. Continuous duty periods of residents should not exceed 16 hours in duration
  16. Minimum Time Off between Scheduled Duty Periods
  17. Residents should have 10 hours (but must have at least eight hours) free of duty between scheduled duty periods.
  1. Matriculation: Requirements for Certificate of Completion for the PGY1 Residency Program

To be considered a graduate of the VA WNY PGY2 Psychiatric Pharmacy Residency Program, the following requirements must be met:

Satisfactory performance in ALL required rotations

At least 90% of all assigned goals and objectives must be achieved

The resident research project must be completed (including completion of a manuscript and submission to an appropriate journal)

All Psychiatric Pharmacy Practice assignments must be completed

At least satisfactory performance in 2 elective rotations

Completion of 2080 hours of residency training

The RPD is responsible for assuring that all of the above are complete before awarding the PGY2 Psychiatric Pharmacy Residency Program graduation certificate to the resident.

  1. Pharmacy Resident Probation/Dismissal and/or Withdrawal Policy:

A resident may be terminated,upon the recommendation of the Residency Committee, at the discretion of the Pharmacy Service Manager for the failure to meet program goals/objectives or planned duration as outlined in this text or for failure to meet the terms of employment of the VA Western New York Healthcare system facilities. These could include excessive AWOL practices, substance abuse, mental impairment, harassment, theft of government property and/or inappropriate professional conduct. Any termination will follow standard VA HR practices of progressive discipline. VHA handbook 5005 (hiring) and VHA handbook 5021 (termination)
