Cuthbertson High School PTSO Scholarship Application

The PTSO is proud to offer each academic year four $1,000 scholarships to seniors pursuing full time higher education after graduating from Cuthbertson High School. The scholarships may be revised (in quantity and amount) based on the fundraising results for the academic year.

The Scholarship Committee is comprised of members representing the PTSO board, teachers and parents. Members may not have children who are seniors at CHS or teach classes with a significant representation of seniors.

The decision of the committee is final and will be announced at Awards Day in May.

Eligibility Requirements:

●  Applicant must be a senior at Cuthbertson High School graduating in June of the current academic year.

●  Minimum weighted GPA of 3.0. GPA will be verified with guidance office. No transcript required.

●  Student or student's parent/guardian must be a paid member of the CHS PTSO as of March 31st in the current academic year. Verify status by contacting Membership Co-Chairpersons: Lori Manalis at or Allison Matthews at .

●  A parent or guardian of each recipient shall be required to guarantee the return of any CHS PTSO Scholarship money received if the recipient does not attend a post-secondary accredited educational institution during the academic year that begins in the fall immediately following his or her CHS graduation.

Application Directions:

●  Submit application and all related documents in an envelope marked PTSO SCHOLARSHIP to the Front Office by 3p.m. on April 14, 2017. Late applications will not be accepted.

●  Complete the attached application in its entirety. Incomplete applications will not be considered. Questions should be directed to: Shannon Mallozzi at or 631-804-2888.

●  Two teacher recommendations must be submitted with the application. Each recommendation must be in a sealed envelope with the teacher's signature over the flap.

●  Essay questions can be attached on a separate piece of paper.

●  Applicant must provide evidence that the student will attend a post-secondary accredited educational institution on a full time basis after graduation. (i.e.: Acceptance Letter, Registration Confirmation, etc.)
