RE: Non - Immersion Mathematics Teacher Mentor (MTM) Application for Region 10 ESC TRC Mathematics Collaborative
By applying for the position of Mathematics Teacher Mentor (MTMs), I understand that I am required to complete the following activities once accepted into the collaborative. Failure to complete any of the following requirements may result in the forfeiture of materials and/or technology paid for by the Region 10 Mathematics Collaborative.
1) I, ______, must obtain a minimum of 100 contact hours over a period of one year from July 28, 2014 through June 15, 2015.
2) As part of this 100 contact hour requirement, I, ______, must attend the Summer Institute, which will occur Monday through Friday from July 28, 2014 to August 8, 2014 and the Wrap Up Day on June 29, 2015 (for 76 contact hours). I am also required to take a pre- and post-assessment on the first day of the Summer Institute and on the Wrap Up Day.
3) I, ______, must complete an additional 12 hours of elective professional development from the list of approved Region 10 face-to-face and/or online trainings provided between July 28, 2014 through June 15, 2015. This additional professional development must be selected from the approved list of courses as outline by the program. (A final staff development schedule will be provided by email once accepted.) I must submit documentation to the project director of the trainings attended on October 1, 2014, January 15, 2015, May 1, 2015, and June 30, 2015.
4) As part of this 100 contact hour requirement, I, ______, am required to mentor other teachers (cadre members) at my campus for a minimum of 12 contact hours each. I must submit documentation of mentoring hours on October 1, 2014, January 15, 2015, May 1, 2015, and June 30, 2015.
5) I, ______, must provide the following data to the Region 10 Mathematics Project Director no later than June 30, 2015.
- Number of students taught by the mentor
- Number of students taught that take the state assessment in Mathematics for 2015
- Number of students taught who did not meet standard on the state assessment in 2015
- Number of students taught who did meet standard on the state assessment in 2015
This data will be submitted through a secure Internet portal, coded by each teacher’s UTEID, reported only in aggregate form (combining data across approximately 1,000 Teacher Mentors) and will not be made available on an individual basis to anyone outside the TRC evaluation staff.
