LIABILITY TERMS & STATEMENT: This sign up form MUST accompany athlete to event.
Your signature above acknowledges your agreement to let your athlete(s) participate in the event above on the dates above, under the clear liability terms below.
All Parents MUST inform School Coaches of any medical conditions your athlete may have.
It is understood that there are physical risks involved in participation baseball sporting events. Reasonable care will be taken to protect athletes, however it is understood and agreed upon that neither:
Event Sponsors, VSI Baseball, Coach Preston Peavy, Coach Mark Campbell or North Florida Baseball Academy nor any assistant coaches, any agents or representatives of the above assume any liability for accidental injury while participating in this event and you agree you will not hold any of the parties responsible for accidental injuries that could occur as a result of participation in this sporting event.
Further, Parents or Guardians agree that you have adequate medical insurance to cover your participant in case of accidental injury and can provide proof of same upon request.
MORE TERMS OF BASEBALL SCHOOL
This event will be held on January 22-23, 2005 according to schedule on page 1.
No refunds for missed events except for family emergencies or genuine illnesses.
SPECIAL NOTE:
In the Event of Inclement Weather or if minimum numbers of students do not sign up for event & event must be rescheduled, refunds are NOT initially made as it is understood that event will be rescheduled. Athlete & parents may choose from the following options;
1) Athlete may signup for the next future hitting or pitching event in Jacksonville to be announcedat no extra charge, even if subsequent event is more expensive.
2) Athlete may turn his school fee into a Personal VIDEO Hitting or Pitching Instruction Voucher for partial credit for a Video Motion Analysis with Kinetics at the event special prices for personal hitting or pitching instruction with the NFBA and VSI Baseball & have the NFBA schedule a time to shoot the HIGH SPEED Video for this analysis according to their schedule. 3) In the event that a suitable time and date for a future event cannot be established within 2 months of planned event then refunds will be issued upon request.
