SPECIFICATIONS OF SERVICE FOR

SUFFOLK COUNTY COMMUNITY COLLEGE

1.THE VENDOR WILL TREAT THE FACILITIES AS REQUIRED. THE DAYS AND TIMES FOR THE SERVICE WILL BE ARRANGED BETWEEN VENDOR AND THE CUSTOMER AND WILL NOT INTERFERE WITH THE NORMAL OPERATION OF THE FACILITIES.

2.THE VENDOR WILL PROVIDE CUSTOMER WITH THE LABELS AND MSDS OF ALL PESTICIDES TO BE USED.

3.DURING EACH SERVICE THE TECHNICIAN WILL PROVIDE CUSTOMER WITH A SERVICE TICKET AND A FLOOR INSPECTION REPORT.

4.VENDOR WILL TREAT ALL THE FACILITIES OCCUPIED BY THE FOOD SERVICE DEPARTMENT. THESE INCLUDE THE KITCHEN, DISHWASHING AREA, DINING ROOM, STAFF LOCKER ROOMS AND RESTROOMS, SERVICE AREA, VENDING MACHINES, FOOD STORAGE, DRY STORAGE, GARBAGE DISPOSAL AREA AND THE OFFICE AREA.

5.VENDOR WILL TREAT FACILITIES FOR BOTH INSECTS AND RODENTS. THESE INCLUDE GERMAN ROACHES, AMERICAN ROACHES, ORIENTAL ROACHES, COMMON HOUSEHOLD ANTS SUCH AS PAVEMENT AND CITRONELLA ANTS, MICE, RATS AND FLYING INSECTS. IF A WASP PROBLEM DEVELOPS DURING THE SUMMER MONTHS, THE TECHNICIAN WILL TREAT THE AREA AT NO ADDITIONAL COST, PROVIDED NO LADDER OR ANY OTHER SPECIALTY EQUIPMENT IS NEEDED.

6.VENDOR WILL TREAT FOR THE DIFFERENT PESTS AS FOLLOWS:

PESTICIDE PRODUCTS AND USE

THE CONTRACTOR SHALL BE RESPONSIBLE FOR THE SAFE USE OF PESTICIDES. ALL PESTICIDES USED BY THE CONTRACTOR MUST BE REGISTERED WITH THE EPA AND APPROPRIATE STATE AND/OR LOCAL JURISDICTION. TRANSPORT, HANDLING, AND USE OF ALL PESTICIDES SHALL BE IN STRICT ACCORDANCE WITH THE MANUFACTURER’S LABEL INSTRUCTIONS AND ALL APPLICABLE FEDERAL, STATE AND LOCAL REGULATIONS. THE ENVIRONMENT AND THE PUBLIC SHALL BE PROTECTED AT ALL TIMES.

THE CONTRACTOR SHALL MINIMIZE THE USE OF SYNTHETIC/ORGANIC PESTICIDES WHEREVER POSSIBLE. FOR EXAMPLE:

1.THE USE OF CRACK AND CREVICE APPLICATION OF PESTICIDES TO PEST HARBORAGE AREAS RATHER THAN FAN SPRAYING EXPOSED SURFACES IN THE GENERAL VICINITY OF HARBORAGE AREAS.

2.THE USE OF CONTAINERIZED BAIT FOR COCKROACHES RATHER THAN SPRAYS WHEREVER APPLICABLE.

3.PESTICIDE FOGS OR SPACE SPRAYS (INCLUDING MISTS AND ULTRA-LOW VOLUME APPLICATION) WILL BE RESTRICTED TO UNIQUE SITUATIONS FOR WHICH NO ALTERNATIVE MEASURES ARE PRACTICAL AS DEEMED NECESSARY AND REQUESTED BY THE USING AGENCY.

PRIOR TO PERFORMING A SPACE SPRAY TREATMENT, THE CONTRACTOR SHALL SUBMIT A WRITTEN REQUEST FOR APPROVAL TO THE USER AGENCY REPRESENTATIVE AT LEAST FIVE (5) DAYS PRIOR TO THE PROPOSED TREATMENT TIME. THE REQUEST MUST IDENTIFY THE SPECIFIC PEST, TIME AND SPECIFIC PLACES(s) OF TREATMENT, PESTICIDE(s) TO BE USED, METHOD OF APPLICATION, PRECAUTIONS TO BE TAKEN TO ENSURE THE CONTAINMENT OF THE SPRAY TO THE SITE OF APPLICATION. NO SPACE APPLICATION OF PESTICIDE SHALL BE MADE WITHOUT THE APPROVAL OF THE USER AGENCY REPRESENTATIVE. NO SPACE APPLICATION SHALL BE MADE WHILE TENANT PERSONNEL ARE PRESENT. NO PRODUCT IDENTIFIABLE AS A FUMIGANT SHALL BE USED IN ANY COUNTY BUILDINGS FOR ANY PURPOSES.

RODENT CONTROL

SNAP TRAPS AND TRAPPING DEVICES (INCLUDING GLUE BOARDS) USED IN RODENT CONTROL MUST BE CHECKED DURING EVERY SERVICE CALL. RODENTS TRAPPED SHALL BE DISPOSED OF BY THE CONTRACTOR WITHIN 24 HOURS AFTER NOTIFICATION BY THE USING AGENCY. TRAPPING SHALL NOT BE PERFORMED DURING PERIODS WHEN MAINTENANCE WILL BE DELAYED BY HOLIDAYS, WEEKENDS, etc. TRAPS SHALL BE PLACED OUT OF THE GENERAL VIEW AND LOCATED SO AS NOT TO BE AFFECTED BY ROUTINE CLEANING PROCEDURES.

ALL RODENTICIDES REGARDLESS OF PACKAGING, SHALL BE PLACED EITHER IN LOCATIONS NOT ACCESSIBLE TO CHILDREN, PETS, WILDLIFE AND DOMESTIC ANIMALS OR IN EPA APPROVED TAMPER RESISTANT (OFTEN TERMED TAMPER-PROOF) BAIT BOXES. FREQUENTLY, BAIT BOX SERVICING SHALL DEPEND ON THE LEVEL OF RODENT INFESTATION. ALL BAIT BOXES SHALL BE PLACED AND MAINTAINED IN ACCORDANCE WITH EPA REGULATIONS WITH AN EMPHASIS ON THE SAFETY OF NON-TARGET ORGANISMS. THE FOLLOWING THREE POINTS SHALL BE STRICTLY ADHERED TO:

1.THE LIDS OF ALL BAIT BOXES MUST BE SECURELY LOCKED OR FASTENED SHUT.

2.BAIT MUST ALWAYS BE PLACED IN THE BAFFLE-PROTECTED FEEDING CHAMBER OF THE BOX AND NEVER IN THE RUNWAY OF THE BOX.

3.ALL BAIT BOXES MUST BE SECURELY ATTACHED OR ANCHORED TO THE FLOOR, GROUND, WALL, etc. SO THAT THE BOX CAN NOT BE PICKED UP OR REMOVED.

THE CONTRACTOR SHALL BE RESPONSIBLE FOR PICKING UP AND DISPOSING OF ALL RODENT CARCASSES IN AND AROUND BUILDINGS LISTED IN THIS BID WITHIN 24 HOURS OF BEING NOTIFIED BY THE USER AGENCY REPRESENTATIVE.

ANT & ROACH CONTROL

ONLY BAITS AND GELS ARE TO BE USED FOR THE CONTROL OF INTERIOR BUILDING INSECTS TO INCLUDE BUT NOT BE LIMITED TO ROACHES AND ANTS.

MONITORING DEVICES AND BOARDS WILL BE USED IN CONJUNCTION WITH BAITS AND GELS TO IDENTIFY

PROBLEM AREAS AND TRACK INFESTATION AND TREATMENT IN THE CONTROL OF INTERIOR PESTS.

MONITORING DEVICES USED IN IDENTIFYING INFESTATIONS AND COUNTS MUST BE CHECKED AND COUNTED AND REPLACED UPON EACH VISIT TO THE SITE UNLESS THE CONTROL OF THE INFESTATION CANNOT BE CONTROLLED IN SUCH A MANNER. IF THIS IS THE CASE, PLANT OPERATIONS SHALL BE NOTIFIED OF THE INFESTATION, PRODUCTS USED AND MEANS OF TREATMENT NEEDED TO FURTHER CONTROL THE INFESTATION.

ALL INSECTICIDES (BAITS & GELS), UNLESS APPLIED IN CRACKS AND CREVICES, WILL BE PLACED IN TAMPER RESISTANT BAIT STATIONS. STATIONS WILL BE PLACED OUT OF THE GENERAL VIEW AND LOCATED SO AS NOT TO BE AFFECTED BY ROUTINE CLEANING PROCEDURES.

OUTDOOR BEES, WASPS, HORNETS, YELLOW JACKETS, ETC.

WHENEVER POSSIBLE, OUTDOOR FLYING INSECT CONTROL WILL BE ACCOMPLISHED BY NON-PESTICIDE MEANS THAT SHALL INCLUDE BUT NOT BE LIMITED TO THE FOLLOWING:

MECHANICAL REMOVAL OF NESTS AND HIVES, THE USE OF NON-PESTICIDE FREEZE SPRAYS, JUICE/JAR

TRAPS WITH BAIT FOR YELLOW JACKETS AND WASPS, ETC.

DEFINITION

FOR THE PURPOSE OF THIS CONTRACT, THE FOLLOWING TERMS AND DESCRIPTIONS SHALL DEFINE THOSE INSECTS THE CONTRACTOR IS TO CONTROL:

  • ROACHES OF ALL SPECIES
  • SILVERFISH
  • EARWIGS
  • ANTS AND ALL CRAWLING INSECTS
  • ANTHROPODS SUCH AS BUT NOT LIMITED TO SPIDERS, TICKS, CENTIPEDES, MITES, ETC.

CONTRACTOR SHALL BE LICENSED BY THE COUNTY OF SUFFOLK AND LOCAL MUNICIPALITIES WHERE REQUIRED.

ALL WORK SHALL BE IN ACCORDANCE WITH THE NATIONAL CODE, CURRENT EDITION, AND ALL LOCAL CODES.

VENDOR MUST FOLLOW THE PROVISIONS OF SUFFOLK COUNTY CODE SECTION 380-5 (E) RELATED TO PESTICIDE APPLICATION ON COUNTY AND COLLEGE OWNED PROPERTIES. (ATTACHED)

SUFFOLK COUNTYCOMMUNITY COLLEGE RESERVES THE RIGHT TO ASSIGN ITS PERSONNEL TO ASSIST CONTRACTOR’S MECHANICS IF THEY DEEM IT TO BE IN THEIR BEST INTEREST.

THE CONTRACTOR SHALL BE REQUIRED TO RESPOND TO SERVICE REQUEST WITHIN TWENTY-FOUR (24) HOURS, INCLUDING WEEKENDS AND HOLIDAYS.

CONTINUAL BREAKDOWN OF VEHICLES IS NOT AN ACCEPTABLE REASON FOR NON-COMPLIANCE, AND SHALL BE A REASON FOR CANCELLATION OF ANY CONTRACT.

NO TRAVEL TIME WILL BE PAID. PAYMENTS WILL BE MADE ONLY FOR THE TIME ON THE JOB. INVOICES MUST BE ACCOMPANIED BY DAILY SERVICE TICKETS SPECIFYING TIME OF ARRIVAL, WORK COMPLETED, MATERIALS USED AND TIME OF DEPARTURE FOR EACH EMPLOYEE, AND SUCH TICKETS MUST BE SIGNED BY AN AUTHORIZED REPRESENTATIVE OF THE COLLEGE. A LEGIBLE COPY OF THIS TICKET SHALL BE LEFT WITH THE SIGNER/REPRESENTATIVE AND SHALL BE THE BASIS FOR ALL PAYMENTS UPON SUBMITTAL OF AN INVOICE.

UNDER THIS CONTRACT SUB-CONTRACTING WILL NOT BE PERMITTED WITHOUT PRIOR APPROVAL OF THE COLLEGE.

THE CONTRACTOR WILL BE REQUIRED TO SUBMIT TO THE COLLEGE COPIES OF AGREEMENTS WITH ALL SUB-CONTRACTOR’S INCLUDING INSURANCE CERTIFICATES. WORK MUST BE DONE IN ACCORDANCE WITH ALL RELEVANT CODES.

A CONTRACT RESULTING FROM THIS BID MAY BE CANCELED BY THE COLLEGE WHEN, IN THE OPINION OF THE ADMINISTRATIVE DIRECTOR OF BUSINESS OPERATIONS, THE SERVICE RENDERED IS INCOMPETENT, INADEQUATE OR UNRESPONSIVE TO THE COLLEGE’S NEEDS.

THE CONTRACTOR SHALL PAY HIS EMPLOYEES THE “PREVAILING RATE OF WAGE” AS DEFINED IN SECTION 220 OF THE NYS LABOR LAW, SCHEDULE OF WAGE RATES AND A COPY OF THE CERTIFIED PAYROLL MUST ACCOMPANY ANY REQUEST FOR PAYMENT.

THE FOLLOWING ATTACHED FORM MUST BE COMPLETED AND RETURNED WITH YOUR BID. FAILURE TO DO SO MAY BE CAUSE FOR DISQUALIFICATION.
