This fillable form is to be used by all CU students on optional practical training (OPT) or academic training (AT), to report a change of address and employment within 10 days of such changes. F-1 Students who are on the 17-month STEM extensions must report to the International Services (IS) office every six months, confirming their information, even if there have been no changes. A new OPT rule imposes limited periods of unemployment to maintain status. The limit is 90 days for students on post-completion OPT. Students with a OPT extension are given an additional 30 days of unemployment time for a maximum of 120 days.

Please note: You must inform IS at the beginning of your authorized employment, so we can validate your employment start date.
