COMPETENCY BASED LIFE SKILLS ASSESSMENT

POINTS TO REMEMBER:

  • Conduct a Strengths and Needs Assessment (within 10 days of admittance to program)
  • Show tool to youth and explain purpose and procedure.
  • Paper tool, used to build rapport, conversation.
  • Identify strengths and then the needs for each section.
  • Always start with Special interests and hobbies.
  • Write down one or two words the youth says on the Strengths and Needs Reporting form..
  • Review document for clarity with youth.
  • Identify 2-3 needs the youth would like to work on.
  • Change needs statement into a SMART Goal.
  • DO NOT GIVE TO YOUTH TO FILL OUT!!!!!!!!
  • Conduct the Casey Life Skills Assessment (within 30 days of admittance to program)

Staff member should complete this part:

  • Go to website
  • Click on the assessment tab
  • Click on CLSA-English
  • Click on the Assessment level you want Child/Youth to take
  • The system automatically defaults to the CLS for youth 14 and older
  • CLSA Level I (8-9)
  • CLSA II (10-12)
  • Please Note: we are only Using option 1- Youth takes the assessment on the Caseworkers/Clinicians Computer or the assessment is printed out and when the youth has finished the assessment, the information is input into the computer by caseworker/clinicians.
  • Complete demographic Youth Information etc.
  • Youth ID: This is automatically assigned once you create a new youth account
  • Org ID: YCStraining
  • Email: (you can add up to 3 additional people to receive reports via email)

9.Be sure to verify email

  • We do not have a special code, just click on next
  • Once youth/caregiver has completed the assessment and clicks on I’m finished a score report will pop up. Print a copy of the report and place in clients chart.
  • The caregiver can complete their portion of the assessment once the youth has completed their assessment.
  • To get a matching Score Report the assessments must be completed within 42 days of each other.
  • Use the information on the Casey life skills website (Under the Help and Training link on top right hand corner of website)
  • Portfolio Assessment:
  • Done during instruction with the youth
  • Used to appreciate, evaluate and assess life skills instruction.
  • Decide on Purpose of Portfolio
  • Decide what will be collected in the portfolio (collection)
  • Decide how items will be selected for inclusion in portfolio (selection)
  • Have youth reflect on each piece as they add it to their portfolio (reflection)
  • Use the items in the youth’s portfolio to rate the youth’s mastery of core skills.(evaluate)
  • Meet with the youth regularly to review his/her work (conference)
  • Celebrate their accomplishments (celebration)
  • Performance Assessment:
  • Direct observation of one’s work
  • Occurs during and just after instruction takes place
  • Recorded on Observation sheets
  • Individual Progress Report
  • Individual OR Group Instruction Report
  • Achievement recorded as :
  • Got It
  • Getting close
  • Attempted
  • Needs Guidance
  • Helps youth appreciate what was demonstrated

The Process:

  • Utilize the completed life skills assessment to address life skill needs in the client’s treatment plan and Joint Care Review.
  • Life Skills instruction should be planned with the youth’s input.
  • Review the treatment plan monthly to assess the progress of the objectives.
  • During Instruction:
  • Portfolios are created
  • Performance is observed and recorded
  • Achievement is recorded
  • At the end of Instruction:
  • Portfolios are reviewed
  • Observations are shared and discussed
  • Thirty (30) days prior to a planned discharge, have the youth complete the Casey Life Skills assessment again to measure life skills learning.
