The purpose of participation in any Authority activity is to have fun. Learning and sportsmanship should be placed ahead of winning. Each player, coach, parent, official and spectator is expected to remember that being involved and encouraging the development of every participant is our goal. By filling out this form each player and parent acknowledges that they will act in a sportsmanlike manner and will at all times conform to the rules of the Authority.
- The Floyd/Floyd County Public Recreational Facilities Authority does not discriminate on the basis of race, color, national origin, age, religion, political affiliation, handicapping conditions, or sex in its recreational programs or employment. No per shall be denied recreational activity or employment solely because of any impairment, which is unrelated to the ability to engage in activities in the position for which application has been made.
- When team rosters are finalized and assigned to coaches ABSOLUTELY NO CHANGES will be made. If you have any concerns contact the Director as soon as possible.
- Fees: A reduced participation fee for families within the same household with more than one child participating in the same sport or sports that may be run simultaneously is available. Please contact the recreational office for more details.
- Players are to be governed by the Rules & Regulations of this league. Players are expected to participate in all of the practice sessions and games scheduled for their team, unless properly excused. Players will not play on any other team/league during the life of this agreement until such action is taken by the proper authorities of this league for their release.
- Parents should be advised that they will be held financially responsible for the loss, misuse, or neglect of any uniform and equipment issued to their child for participation in the youth league.
- The Floyd/Floyd County Public Recreational Facilities Authority DOES NOT carry medical insurance on participants. Each child should have separate medical insurance for injuries, which may be sustained due to this activity.

Please send separate checks for each child participating and attach to appropriate form.
Make checks payable to Floyd Recreational Authority. If contract form, fee and birth certificate are not returned together, application will not be accepted. Call the office for refund information.

LEAGUE INFORMATION ON BACK
LOCAL LEAGUE AGE REQUIREMENTS:
COED T BALL:5, 6 YEAR OLDS. Must be five years old before May 1st of the current calendar year.
**Dixie Softball changed their cut off age from May 1st to January 1st.
Your league age will be your age prior to January 1st of the current calendar year.
Due to this we will have a transition year for 2009.
Since T- Ball has a cut off date of May 1st, girls leaving T Ball and entering the Darlings Division who will be turning seven years old between January 1st and before May 1st , 2009, can choose to participate in T-Ball or the Dixie Darlings division.
Softball:
DARLINGS:7, 8 YEAR OLDS. Anyone whose seventh (7th) birthday falls before January 1st of the current calendar year must play in this division.
DIXIEANGELS:9, 10 YEAR OLDS. Anyone whose ninth (9th) birthday falls before January 1st of the current calendar year must play in this division.
DIXIE PONYTAILS:11, 12 YEAR OLDS. Anyone whose eleventh (11th) birthday falls before January 1st of the current calendar year must play in this division.
DIXIE BELLES:13, 14, 15 YEAR OLDS. Anyone whose thirteenth (13th) birthday falls before January 1st of the current calendar year must play in this division.
DIXIE DEBS:16, 17, 18 YEAR OLDS. Anyone whose sixteenth (16th) birthday falls before January 1st of the current calendar year must play in this division.
Baseball:
DOUBLE A:7, 8 YEAR OLDS. Anyone whose seventh (7th) birthday falls before May 1st of the current calendar year must play in this division.
DIXIE YOUTH MINORS: 9, 10 YEAR OLDS. Anyone whose ninth (9th) birthday fall before May 1st of the current calendar year must play in this division.
DIXIE YOUTH MAJORS: 11, 12 YEAR OLDS. Anyone whose eleventh (11th) birthday falls before May 1st of the current calendar year must play in this division.
DIXIE BOYS:13, 14 YEAR OLDS. Any boy whose thirteenth (13th) birthday falls before May 1st of the current year and any boy whose fifteenth (15th) birthday falls on or after May 1st of the current year is eligible to compete and complete the current season.
DIXIE PRE MAJORS:15, 16 YEAR OLDS. Any boy whose fifteenth (15th) birthday falls before May 1st of the current calendar year and any boy whose seventeenth (17th) birthday falls on or after May 1st of the current calendar year is eligible to compete and complete the current season.
DIXIE MAJORS:15 to 19 YEAR OLDS. Any boy whose fifteenth (15th) birthday falls before May 1st of the current calendar year and any boy whose nineteenth (19th) birthday falls on or after January 1st of the current calendar year is eligible to compete and complete the current season.
