AFTER SCHOOL ATHLETICS’ STANDARDS AND POLICIES

ACADEMICS

o  To be eligible to play, student athletes must have and maintain a 2.0 GPA.

o  Student athletes are required to participate in their Physical Education classes to be eligible to participate in after school athletics, this includes practices, travel, games, and end of season celebrations.

1)  If a student athlete is excused from participating in Physical Education classes, they will not be allowed to participate in after school athletics until they have been cleared.

GRADE CHECK PLAN

o  All sports will have one grade check in middle of intramural and traveling team practice

seasons. All students not previously eligible can be made eligible at this grade check.

All students previously eligible can also be made ineligible at this grade check.

o  Grade checks not returned on time will result in a 0.0 GPA

o  If a team makes playoffs, there will be a grade check prior to playoff/championships. This grade check is to ensure eligibility for players representing our school at post season.

BEHAVIOR/DISCIPLINE POLICY

All student athletes who receive discipline action (On campus opportunity, After school detention, Friday school) will be exempt from participating in athletics for that day (practices, games)

DETENTIONS

1) Detentions disqualify you from participation in both practice and games for that day.

2) Detentions will be served the day assigned. Changing day of detention to allow

participation in athletics is prohibited.

3) 3 strikes policy. On the 3rd detention during season, the AD and Administration will

review the student athlete’s behavior to determine if he or she will return to participate

with that team.

SUSPENSIONS

1) ALL suspensions are subject to removal from athletic participation. The Athletic Director will, along with Administration, review each suspension to determine if that student will return to participate with his or her athletic team. Return to athletics will be a contractual basis only

UNIFORMS

o  Uniforms are provided for traveling teams only. Intramurals are not uniformed.

o  The cost to replace uniforms are as follows:

o  -Jerseys $50.00

o  -Shorts(if issued) $40.00

o  -Kneepads (if issued) $30.00

o  Uniforms are property of Monte Vista Middle School.

o  Student athletes are responsible for the uniforms they are issued.

1) Students will be charged to replace uniforms if:

a. Uniform is found unnecessarily worn out

b. Uniform has not been kept well and is not usable

c. Uniform has gone missing

·  Uniforms are due back to coaches at the end of the season. If uniforms are not turned

in, the student athlete will not be allowed to participate in school activities, including

field trips, until the uniform has been paid for or returned.

TRANSPORTATION

o  Monte Vista Middle School will provide transportation to and from athletic events, with the exception of athletic events held at North Mountain Middle School.

o  All games held at North Mountain Middle School are ONE WAY ONLY. Transportation

from North Mountain Middle School will be provided by parent.

o  Students are required to use the mode of transportation provided by Monte Vista Middle

School, unless otherwise excused through a Transportation Agreement.

o  Athletic Coaches will maintain a sign out log for parents who wish to drive their student

athletes after a game.

o  Only Adults are allowed to sign out a student athlete.

o  Student Athletes will only be released to parents/legal guardian, or approved adults.

o  Any student athlete who is not properly signed out, may be removed from athletic team.

PARENT/LEGAL GUARDIAN ACKNOWLEDGEMENT

AND HOLD HARMLESS AGREEMENT

I understand that if my student athlete is found in violation of these standards, he/she will be subject to the discipline procedures, which may result in his/her removal from a team or activity. I also understand that all sports can involve MANY RISKS OF INJURY including, but not limited to, death, serious neck and spinal injuries which may result in complete or partial paralysis, brain damage, serious injury to virtually all internal organs, serious musculoskeletal system, and serious injury or impairment to other aspects of his/her body, general health, and well-being. I understand that the dangers and risks of playing or practicing to play/participate in sports may result not only in serious injury, but in a serious impairment of my student athlete’s future abilities to earn a living, to engage in other business, social and recreational activities, and generally to enjoy life.

In consideration of the San Jacinto Unified School District permitting my student athlete to try out for the Monte Vista Middle School after school athletic team(s) I have circled below, and to engage in all activities related to the team(s) including, but not limited to, trying out, practicing, playing/participating, and traveling, I agree to hold the School District, it’s employees, agents, representatives, coaches, and volunteers harmless from any and all liability, actions, causes of action, debts, claims, or demands of every kind and nature whatsoever which may arise by or in connection with participation of my student athlete in any activities related to the Monte Vista Middle School after school athletic team(s) circled below.

TRANSPORTATION AGREEMENT

However, at times I will want/have the need to provide transportation for my student athlete and will wish to deny transportation provided by the District.

IF I, OR SOMEONE I APPROVE, PROVIDE TRANSPORTATION OUTSIDE THE MODE OFFERED BY THE SAN JACINTO UNIFIED SCHOOL DISTRICT, IT IS FULLY UNDERSTOOD THAT THE DISTRICT IS IN NO WAY RESPONSIBLE, NOR DOES THE DISTRICT ASSUME LIABILITY, FOR ANY INJURIES OR LOSSES RESULTING FROM THIS NON-DISTRICT SPONSORED TRANSPORTATION.
