Job Description
Job Purpose
To provide an efficient support service to the Forensic Mental Health Practitioner (FMHP)Service and the wider resources as part of the Brent Offender Health Hub. To effectively interface with Together, the FMHP Service and key external stakeholders. To carry out these duties in the context of Together’s value system and Code of Conduct.

Key Accountabilities
Systems and procedures
- Develop, maintain, review and improve relevantadministrative, data collection and evaluation systems and services, to ensure all requirements (internal and external) are met.
- Evaluating and monitoring the FHMP Service through the coordination and collation of information submitted and dissemination to stakeholders
- To track client journeys through the collation of quantative and qualitative data.
External Relationships
•Manage external relationships through co-ordination of referrals, meetings and management of single points of contact
Managing resources
- Integrate, prepare and supply information from a variety of sources to produce reports as required.
- Manage external relationships through co-ordination of referrals, meetings and management of single points of contact
- Maintain and develop secure and confidential records
- Contribute to scheduling and co-ordinating activities and resources by maintaining database of relevant stakeholders, room bookings, stationery and marketing materials

Development
Support new service developments and assist with bid and tender preparations, as required.
Confidentiality
- Information relating to users must be treated in the strictest confidence and discussed only within the project with the appropriate members of staff or with named assessor/care co-ordinator.
- Information relating to staff must be treated in the strictest confidence and discussed only with other senior staff or line manager.
