To all Residents, Fellows, and Candidates that are planning to submit (or resubmit) Credentials for 2018.
Attached please find the 2018 Credentials Application Packet, which contains all necessary forms and detailed instructions. This packet must be used for all submissions.
A copy of the standard recommendation form (part of the packet) is also included for easier use. The packet and recommendation form will also be posted on the ACVECC website (www.acvecc.org); please do not fill out the forms online; the packets should be downloaded to your hard drive, filled out, and then printed.
Remember that online submission of (4) multiple choice questions is also required by January 15th. Instructions are included on page 10 in the packet and online at: www.acvecc.org/exam.
It is important that you give yourself enough time to develop and successfully submit your questions; a minimum of 2-3 weeks may be necessary.

There have been some changes in the application process in the last several years:
1. The Nov 1st deadline for manuscript submission has been eliminated; however, we still recommend that manuscripts be submitted as early as possible to allow adequate time for completion of the review process by June 30th. Manuscripts must be submitted by January 15th.
2. All submissions will be electronic in a single PDF submitted by email to the Office of the Executive Secretary, Kathleen Liard at , and uploaded in to the documents section of the database. A hard copy of the application should be kept by the candidate and can be requested by the Credentials Committee during the review process.
3. Deadlines will be STRICTLY enforced. We ask that mentors remind candidates and candidates remind mentors of all deadlines so everyone can keep on track.
