*To be eligible for mileage and meal reimbursements, participants must have driven over 35 miles one way to attend the workshop.
Complete the information requested above to apply for an RTAP Scholarship. Send this completed Scholarship Application form to the Office of Transit at before the deadline of March 25th, 2016.
Lodging
§ Lodging expenses are reimbursable under the following conditions:
o One day programs if the applicant is traveling 75 miles or more to the event site.
o Multiple day programs if the applicant is located more than 35 miles one way from the event.
o Original receipts showing a zero (0) balance for lodging must be submitted.
Transportation
§ Travel reimbursement will be from corporate address to training event address only.
§ Mileage will be reimbursed for travel to and from events at your agency rate (not to exceed the current MnDOT rate of .54).
§ All mileage reimbursement requests must be accompanied by a copy of a direction finding program, such as MapQuest or Google Maps.
§ Rental car usage must be approved in advance by the RTAP Coordinator.
Other expenses not described above may be reimbursable depending upon individual circumstances. Please discuss these with the RTAP Coordinator.
A Request for Reimbursement form will be sent to applicant(s) when a scholarship is awarded. This must be postmarked with driving verification map(s) to the Office of Transit by May 25th, 2016 to the following address:

MnDOT reserves the right to deny reimbursement requests submitted after the 30 day period. Please allow 30 days to process reimbursement requests.
